Abstract

Background: Radiographic examination of the temporomandibular joint (TMJ) is an important procedure for assessing the structural condition of the joint, particularly in cases of subluxation. At the Radiology Department of RSUD (Regional General Hospital) Banyumas, this procedure is generally performed using the axiolateral projection with the Schuller method in accordance with the standard operating procedure (SOP). However, in practice, an additional AP Axial Towne projection is often used to obtain more comprehensive diagnostic results, especially for patients with a history of trauma or complex complaints. Methods: This study employed a descriptive qualitative approach using a case study method. Data were collected through direct observation, interviews with radiographers and radiologists, and documentation of patient radiographs. During the data reduction phase, collected notes and recordings were transcribed and subsequently coded using open coding techniques. Results: The addition of the AP Axial Towne projection provided enhanced visualization of the osseous structures of the TMJ, particularly the condyle and mandibular fossa. This additional projection allows radiographers to identify abnormalities such as fractures or positional changes of the condyle that may not be clearly visible using the Schuller projection alone. Conclusion: The inclusion of the AP Axial Towne projection in TMJ examinations for clinical subluxation is both effective and clinically relevant. The researchers recommend adding the Law method projection for TMJ assessment, as it uses a minimal beam angulation, reducing distortion, preventing muscle spasm, and providing more accurate anatomical representation. In addition, using a high kV technique is recommended to minimize the radiation dose received by patients.

Abstract

ABSTRACT Lopography examination in pediatric patients with clinical post-colostomy Hirschsprung is performed to obtain images of colonic anatomy and contrast-medium distribution as a basis for evaluating the patient’s condition before further management. This study aimed to describe the lopography examination procedure and the rationale for not performing post-evacuation imaging in pediatric patients with clinical post-colostomy Hirschsprung. A descriptive qualitative method with a case study approach was employed at the Radiology Installation of RSD K.R.M.T. Wongsonegoro Semarang from September 2025 to May 2026. Data were collected through observation, interviews, documentation, and literature review involving three radiographers and one radiologist. The findings showed that patient preparation included data verification, cleaning of the stoma area, removal of the stoma bag, changing clothes, and completion of informed consent. The contrast medium consisted of iodine water-soluble contrast and NaCl at a ratio of 1:4, while imaging was performed using plain AP, AP post-contrast, and lateral projections. Post-evacuation imaging was not performed because the contrast medium was water-soluble and could be expelled from the body, while radiation exposure was also considered in view of the patient’s pediatric age. The examination provided visualization of contrast-medium distribution in the proximal and distal colon and information regarding the colonic condition in the observed case.

Abstract

ABSTRACT Radiographic examination of the humerus in emergency trauma cases generally uses two projections, namely anteroposterior (AP) and lateral projections. However, at the Radiology Department of PKU Muhammadiyah Karanganyar Hospital, a single-exposure technique using an AP chest projection including the humerus is employed. This study aimed to determine the examination procedure, the rationale for using this technique, the effectiveness of the standard operating procedure (SOP), and the implementation of radiation protection measures. This study employed a qualitative descriptive method with a case study approach. The study was conducted at the Radiology Department of PKU Muhammadiyah Karanganyar Hospital from March to April 2026. Data were collected through observation, interviews with three radiographers, one radiologist, and one referring physician, as well as documentation. The data were then analyzed through data reduction, data presentation, and conclusion drawing. Radiographic examination was performed using an AP chest projection including the humerus, with the patient in the supine position and a single exposure. This technique allows the thorax and humerus to be visualized simultaneously, thereby supporting diagnostic assessment and surgical preparation. The single-exposure technique was effective in shortening examination time, reducing patient mobilization, improving patient comfort, and lowering radiation dose in accordance with the ALARA principle. Radiation protection was implemented through appropriate collimation and optimal exposure factors. However, a limitation of this technique is that it does not optimally demonstrate the lateral view of the humerus. The SOP for humeral radiographic examination using an AP chest projection including the humerus with a single exposure is effective for emergency trauma cases because it meets diagnostic needs, improves service efficiency, and optimizes radiation protection. A lateral projection may be added, when necessary, based on clinical indications.
