Abstract

Introduction: Meningiomas of the sphenoid wing make up approximately 15–20% of total cranial meningiomas.Giant medial sphenoid involve the dura of the greater and lesser wings of the sphenoid, the anterior clinoid process,the spheno-orbital bone and the middle cranial fossa, hyperostosis bone was common to found. Surgicalmanagement of giant medial sphenoid is extremely challenging due to their intimate relationship with vital neuralstructures.Case Report : A 40-year-old woman came to the emergency room with complaints of decreased consciousness thathad been experienced since 1 week , recurrent headaches, weakness of the right limb and history of blurred vision. Anon-contrast head ct scan and MRI brain contrast revealed an extra-axial giant mass meningioma in left medialsphenoid wing with hyperostosis bone Craniectomy tumor removal was performed with a subtemporal approachcombined with cranio-orbytozygotomy.Discussion : Hyperostosis is a common phenomenon occuring in different meningiomas with incidence rangingfrom 25 to 49% of meningiomas.it is most commonly seen in sphenoid wing and convexity meningioma . The bonyhyperostosis is of neoplastic nature and is responsible for many of the clinical manifestation of such tumors andhence should be totally drilled to achieve cure and avoid recurrence.Conclusion : Sphenoid wing meningioma en plaque, mainly meningothelial meningiomas, are characterized by theassociated bony hyperostosis that gives them a distinct radiological appearance. Extensive tumor removal is crucialfor correction of proptosis and adequate visual decompression to achieve satisfactory cosmetic and functionaloutcome.

Abstract

Background: Anterior cervical discectomy and autograft fusion (ACDF) is the gold standard of surgical treatment for HNP cervical. However, donor site complication has found in the form of pain that could lead to disability. The edge of the donor site defect can initiate pain may reduce patient’s quality of life. High ambulation can be earned by smoothing the donor site defect. Objective: The purpose of this study is to determine the quality of patient rehabilitation after ACDF with autograft between smoothing group and non-smoothing group. Methods: This cross-sectional study was conducted at the Adenin Adenan Hospital Medan. Patients who had undergone ACDF with autograft in 2015–2020 were assessed for their disability at the first, third, and sixth months postoperatively using Barthel Index. Total sampling method was used with inclusion and exclusion criteria. Results: There were 41 respondents in this study, 20 people in the non-smoothing group and 21 people in the smoothing group. The disability experienced by the smoothing group was less than the non-smoothing group at each month of measurement, but only significant (p<0.05) at the sixth month. Conclusion: Conclusions of this study was disability and rehabilitation outcomes in the smoothing group were better than in non-smoothing group.

Abstract

Progressive Muscle Relaxation (PMR) is a relaxation technique for muscle by tensing or tighteningone certain group of muscles and releasing the tense followed by focusing the mind to feel thesensation of relaxing. This research was aimed at observing the effect of PMR on fatigue symptom ofpeople with Type 2 Diabetes Mellitus (T2DM). Quasi experimental Pretest-Posttest Design withcontrol group was used as the design of this research. The samples were collected by employingconsecutive sampling technique with 66 respondents consisting of 33 people in intervention group andthe other 33 people in control group. The fatigue symptoms observed in this research were measuredby using Piper Fatigue Scale. It was found from the results that there were some significant differenceson the fatigue symptoms before and after giving PMR to both the intervention group with t=43,57,p=<0,001; and the control group with t=1.73, p=0.093. Furthermore, another significant differencewas also found between the intervention and control group which was t= -30.453, and p<0.001.Finally, it was concluded that PMR was effective to reduce the fatigue synptoms of people with T2DM.
