Abstract

Introduction : Meningiomas are benign tumor and the most prevalent intracranial tumors which recur locally. Their morphologic grading do not always correlate with patient outcome and some of them associated with a less favorable clinical outcome. The incidence in women is approximately twice than in men. Progesterone receptors may play role in the proliferation and grading of meningiomas. Aims of this study isto evaluate the correlation of progesterone receptor status with proliferation marker ki67 in various grades of meningioma. Methods : A retrospective research of 30 paraffin-embedded sections of meningiomas was collected that was done histological typing and grading with hematoxylin and eosin. Immunodetection of Progesterone receptors was conducted with peroxidaseantiperoxidase complex and Ki-67 was achieved by streptavidin-biotin peroxidase complex. Progesterone receptors status considered positive if >50% of nuclei were stained with medium intensity or >10 of tumor cell’s nuclei were strongly stained. The Ki-67 is defined into three groups as no proliferation, low proliferation (<14%), and high proliferation (> 14%). Results : Progesterone receptors were positive in 13/27 (48.1%) of grade I tumors, 1/2 (50%) of grade II tumors, and 0/1 (0%) of grade III tumor. In grade I tumor, negative progesterone receptors with negative Ki-67 was 6/14 (42.9%) and low Ki-67 was 8/16 (57.1%). While, positive progesterone receptors with negative Ki-67 was 7/13 (53.8%), low Ki-67 was 4/13 (30.8%), and high Ki-67 was 2/13 (15.4%). In grade II tumor, negative progesterone receptors with negative Ki-67 and positive progesterone receptors with low Ki-67 expression. In grade III tumor, negative progesterone receptors with negative Ki-67 expression. Conclusion : There was a not significant relationship between Ki-67 and progesterone receptors status, but high Ki-67 expression was only found in positive progesterone receptors. Progesterone receptors may play role in proliferation of meningioma. Keywords : meningioma; progesterone; receptor; Ki-67 antige

Abstract

Introduction : Traumatic brain injury (TBI) is the most common problem that caused morbidity and mortality in the world. Epidemiology information of TBI is very important in understanding the causes and the risk factors of TBI, so that effective programs can be designed to prevent injury. This study purposed to provide overview of TBI pattern in tertiary referral hospital at North Sumatera with focus on epidemiological data of TBI pattern. Methods : The design of this study is a retrospective, and it is done in general hospital H. Adam Malik from June-December 2018 with sample from all patient with traumatic brain injury that admited in hospital. All medical records with diagnosis traumatic brain injury will be assessed for several variables as age, gender, etiology, severity, length of stay and outcome. Results : During the period of study, 118 patients with traumatic brain injury were admitted in neurosurgery ward. The highest TBI cases was occurs in male patients with range age between 18-35 years old, and caused by traffic accident. Most patients admitted to Emergency department with GCS 13-15 and epidural hematoma was the most common lesion found in head CT scan. Length of stay in this research mostly between 1-5 days,  and  the mortality rate is about 16.9%, which is dominated by severe head injury. Conclusions : Head injury mostly found in male with productive age, and traffic accident be the most common caused. Severe head injury still have quite high mortality rate.

Abstract

Brain tumors are the most common disease group of solid tumors in childhood, and children with brain tumors have a relatively poor survival rate. Epidemiologic data from a hospital-based registry provide the necessary information to obtain a full picture of the frequency of this disease, which is a great challenge in pediatric oncology. One hundred thirty four tumors in children between 0 and 17 years of age diagnosed between 2013 to 2017 were classified according the sex, and topography. Incidence of brain tumor in boys were higher than girls. From the registry data, the prevalence for boys was higher (67%). In the whole series, 54% were supratentorial, 46% infratentorial. The most common histopathologic pattern seen in the registry were pilocytic astrocytoma which accounts for 42 cases (31%) followed by medulloblastoma and craniopharygioma with the percentage of 25% and 18% respectively. The least histopathologic pattern of pediatric tumor seen in our center are diffuse astrocytoma and choroid plexus papiloma which was only seen in 1 patient for the last 5 years of our experience.

Abstract

Abstract: Ventriculoperitoneal (VP) shunt placement is one of the most commonly performed procedures in neurosurgery. One rare complication is the formation of an abdominal pseudocyst, which can cause shunt malfunction.

Abstract

Traumatic brain injury (TBI) is a common concern for the causes of disability and mortality and it can cause post-traumatic seizure (PTS). Currently, Brain Trauma Foundation (BTF) recommends phenytoin (PHY) as early PTS prophylaxis for patients with severe TBI. The two most common drugs prescribed for PTS are levetiracetam (LEV) and PHY. However, PHY displays a wide array of disadvantages. LEV appears as a better alternative to PHY because of its easier administration, the absence of need for drug level monitoring, lower drug-drug interaction, and better side-effects profile. It is due to the linear elimination kinetics LEV had in comparison to PHY that have zero order pharmacokinetics. Theoretically, LEV is better than PHY. But according to prior studies, LEV and PHY have comparable efficacy at preventing PTS in the early stages. Furthermore, the current evidence is insufficient to definitely recommend LEV over PHY in terms of effectiveness and adverse effects. This study aimed to analyze levetiracetam as an alternative to phenytoin for prophylactic use in post-traumatic seizure.

Abstract

Background: Intestinal parasitic infections are quite a big problem and are still often found in society. Intestinal parasitic infections are caused by infections of intestinal protozoa and intestinal nematodes. Intestinal parasitic infections can result in a decline in an individual's health, nutrition, intelligence and productivity. Personal Protective Equipment (PPE) and good personal hygiene have an important role in reducing the transmission of parasites from soil to humans, both through eggs, larvae and adult parasites, especially for waste workers who have direct contact with infectious environments. In the city of Medan, the final disposal site used is TPA Terjun. Every day, TPA Terjun receives waste from various areas in the city of Medan. Workers are at high risk of being infected by intestinal parasites that spread through the soil. Objective: The purpose of this study is to determine the relationship between the habit of using PPE and personal hygiene with the incidence of intestinal parasite infections in waste workers at TPA Terjun Medan. Methods: This study included 121 subjects from TPA Terjun, Medan. This research used cross-sectional approach. Result: This study found that there was no correlation between the use of gloves, masks, protective clothing and protective shoes on the incidence of intestinal infections in waste workers at TPA Terjun Medan, also between personal hygiene and the incidence of intestinal parasite infections in waste workers at TPA Terjun Medan. Conclusion: The use of PPE is important to prevent the transmission of intestinal parasitic infections to workers at TPA Terjun, Medan.
