Abstract

Background: The prevalence of stroke in Indonesia has been increasing and has had a significant impact on individuals, families, and society. There is a pressing need for innovative assistive devices that support independent therapy and enhance patient motivation to improve post-stroke productivity. This study aimed to design and develop a post-stroke rehabilitation assistive device utilizing electromyography (EMG) technology. Methods: This study employed a research and development (R&D) design. Data were collected through documentation studies, interviews with physiotherapists, observations, and focus group discussions (FGDs). A documentation study was conducted using the medical records of stroke patients. Interviews were conducted with physiotherapists, and observations were carried out to understand patients’ therapeutic activities. Results: This study provided alternative therapy by developing post-stroke rehabilitation aids with EMG technology. Based on the documentation study, 88% of patients experienced ischemic stroke with a good level of consciousness (E4V5M6), indicated by the Glasgow Coma Scale (GCS) score of 15. Conclusion: This phenomenon supported the development of electrical therapy innovations to accelerate stroke recovery. This study successfully designed a hand therapy device using EMG technology to improve patients’ motor function, particularly among those with E4V5M6 levels of consciousness and limited motor skills.

Abstract

Introduction: Psychological support is important for patients with physical disabilities following workplace accidents because psychological problems may affect recovery and rehabilitation. In 2025, 145 workers in Riau Province experienced physical disabilities following workplace accidents. Objective: This study aimed to develop and improve psychological services for patients with physical disabilities following workplace accidents during inpatient care. Method: A descriptive qualitative approach with a research and development design using the ADDIE model was conducted at a hospital in Riau Province in 2026. Data were collected through observation, interviews, document review, and Focus Group Discussions involving relevant healthcare professionals and hospital management. Results and Discussion: The developed psychological service provided a standardized pathway for screening, intervention, multidisciplinary coordination, and follow-up support. The findings show that improving psychological services requires the integration of medical, psychological, social, and occupational aspects, with psychological support continuing after hospital discharge. Conclusion: Standardized psychological services are important for strengthening comprehensive occupational accident rehabilitation, improving multidisciplinary coordination, supporting continuity of psychological care, and facilitating psychological adaptation and return to work.

Abstract

Healthcare facilities in Indonesia are required to report patient safety incidents. However, many hospitals continue to face challenges in integrating incident data, conducting systematic analyses, and using incident reports to inform continuous quality improvement initiatives. The Regional General Hospital of Yogyakarta City faces similar challenges because incident reporting and analysis have not been fully integrated within a single system. Therefore, this study aimed to evaluate the implementation of the Patient Safety Incident Reporting (PaSIR) system and explore its contribution to improving incident reporting practices. This study employed a Research and Development (R&D) approach using a User-Centered Design (UCD) methodology. The qualitative evaluation was conducted between January and June 2025. Purposive sampling was used to recruit10 participants, including members of the patient safety team, information technology staff, and public relations personnel. Data were collected through interviews, surveys, document reviews, and focus group discussions (FGDs). Implementation was evaluated using the Consolidated Framework for Implementation Research (CFIR), and data were analyzed thematically using triangulation. Written informed consent was obtained from all participants, and ethical approval was secured prior to the study. Before implementation, incident reporting was fragmented across paper-based forms, Google Forms, and the hospital information system. PaSIR integrated key incident management process, including reporting, validation, root cause analysis, follow-up, and feedback. The findings indicated that PaSIR was user-friendly, responsive to user needs, and provided a more transparent and integrated workflow for incident management. Key factors supporting implementation included staff engagement, regulatory alignment, cross-functional collaboration, and managerial commitment.  PaSIR improved the effectiveness of incident reporting, strengthened the patient safety culture, and supported data-driven decision-making. Nevertheless, greater managerial coordination, organizational readiness, user engagement, and ongoing technological integration are needed to ensure sustainable implementation. Keywords: Patient Safety, Risk Management, User-Centered Design, Quality Improvement, Electronic Health Records
