Abstract

Ruptur perineum merupakan perlukaan jalan lahir yang terjadi pada saat kelahiran bayi baik menggunakan alat maupun tidak menggunakan alat. Ruptur perineum disebabkan oleh paritas, jarak kelahiran, berat badan bayi, pimpinan persalinan tidak sebagaimana mestinya, ekstraksi cunam, ekstraksi fakum, trauma alat dan episiotomi. Kejadian ruptur perineum berdasarkan paritas paling banyak terjadi pada primipara yaitu sebesar 62% dan berdasarkan berat badan bayi paling banyak pada berat badan >3500 gram yaitu sebesar 46%. Tujuan penelitian ini adalah untuk mengetahui adakah hubungan antara paritas dan berat badan lahir bayi dengan kejadian ruptur perineum di Puskesmas Madukara 2 Kabupaten Banjarnegara. Jenis penelitian yang gunakan adalah survei analitik dengan pendekatan crosss sectional. Populasi  dalam penelitian ini adalah seluruh ibu bersalin di Puskesmas Madukara 2, Kabupaen Banjarnegara. Teknik pengambilan sampel dengan metode consecutive sampling, besar sampel sebanyak 36 ibu bersalin. Teknik analisis menggunakan uji Chi-Square. Hasil penelitian menujukan bahwa 66,7% responden multipara, dan 63.9% memiliki bayi dengan berat badan lahir <3500 gram. Terdapat hubungan antara paritas (p=0,001<0.05, α= 95%), berat badan lahir bayi (p=0,000<0.05, α= 95%) dengan kejadian rupture perineum. Ada hubungan yang bermakna antara paritas dan berat badan lahir bayi dengan kejadian rupture perineum. Ibu diharapkan dapat melakukan ANC secara teratur sehingga taksiran berat badan janin dapat terpantau melalui pengukuran TBJ dan informasi terkait tentang persiapan persalinan seperti bagaimana cara mengejan yang benar dan posisi dalam mengejan.

Abstract

World Health Organization (WHO) estimated 800 women die d every day due to pregnancy complications and the birth process. About 80% of deaths maternal is the result of increased complications during pregnancy, childbirth and breath. Maternal Mortality Rate (MMR) in the world which is 289,000 inhabitants. Almost two thirds Maternal death is caused by direct i.e. bleeding (25%), infection/ sepsis (15%), eclampsia (12%), abortion unsafe (13%), obstructed labor (8%), and other direct causes such as pregnancy ectopic, embolism, and other things related to anesthesia problems (8%), other causes (19%). A preliminary study was conducted at the Banjarnegara 1 Health Center regarding labor pain in the active phase of the 1st stage and it was found that 67% of mothers stated moderate pain and 33% said severe pain. The purpose of this study was to determine the relationship between pain and duration of labor first stage at the Banjarnegara 1 Public Health Center, Banjarnegara Regency. Analytical descriptive research design with a cross sectional approach. The research population was mothers who gave birth at the Banjarnegara Health Center 1. The sampling technique was using a purposive sampling technique as many as 35 respondents. All samples will be given a questionnaire about pain and the duration of labor is calculated using hours. The results showed that more than half the level of labor pain in the first stage of the respondents were in the moderate category, namely 18 respondents (52%), most of the duration of labor in the first stage of the respondents were included in the normal category, namely as many as 24 respondents (68%) and there was a relationship between pain and first stage of labor with p_value 0.018. Things that can be done by health workers to reduce labor pain and duration of labor is to bring birth attendants and be taught deep breathing techniques in addition to reducing anxiety.

Abstract

Infant Mortality Rate (IMR) is calculated from the number of infant deaths 0 <12 months per 1000 live births in an area in one year. The Infant Mortality Rate (IMR) in Banjarnegara Regency in 2018 was 14.1 / 1000 live births, which in absolute terms was calculated from the number of infant deaths of 216 with live births of 15,317. The Infant Mortality Rate (IMR) in 2018 increased compared to 2017 which was only 13.37 / 1000 live births with the number of deaths of 204 cases from 15,255 live births. One of the important obstacles for the progress of MDGs 4 so that it fails to reduce infant mortality, namely death from a single cause, prematurity. Preterm labor is a dangerous thing because of the potential to increase perinatal mortality by about 65% -75%, generally associated with low infant weight weighing less than 2,500 grams. Several factors have contributed to the occurrence of preterm labor such as factors in the mother, fetal and placental factors, or other factors such as socioeconomic. The research objective was to determine the picture of maternal age and parity as risk factors for preterm birth. The basic method of research used in this research is descriptive analysis research method by looking at the mother's age as a risk of preterm birth. The sampling technique was carried out by the total sampling method, namely taking all samples from the total population. The data collection method in this study was carried out through secondary data, namely taking medical records. The results showed that most of the respondents 90.3% (37 people) were in the low risk age category (20-35 years), and there were 9.7% (4 people) in the high risk age category (<20 and> 35 years). More than half of maternal parity was in the low risk category, namely 22 respondents (53.7%) and the other part, namely 19 respondents (46.3%), which were in the high risk category.

Abstract

Child health efforts are expected to reduce child mortality. One of the indicators is the Infant Mortality Rate. The purpose of this study was to determine the description of the factors of asphyxia in newborns at the Banjarnegara Puskesmas 2 Banjarnegara District. This type of quantitative research is analytic survey design, cross sectional approach method. The population is all mothers who give birth to newborns with asphyxia in January - May 2020 at the Banjarnegara Health Center 2 as many as 22 newborns with asphyxia. The sampling technique uses total sampling. Univariate analysis looked at the frequency distribution and the percentage of each variable studied, namely maternal age, maternal parity, gestational age, difficult delivery and amniotic fluid conditions. The results of the study are mothers who give birth to newborns with asphyxia, the majority of those at low risk are 77.27%. Parity of mothers giving birth all included in the category of primipara and grandemultipara by 100%. The gestational age of mothers in all categories of term pregnancy is 100%. The majority of asphyxia occurs in mothers with difficult births in the amount of 90.91%. The condition of amniotic fluid the majority of amniotic fluid mixed with meconium of 68.18%.
