Abstract

Exclusive breastfeeding during the first six months of life is an important strategy to improve children's health, growth, and development. The success of exclusive breastfeeding is not only determined by the mother's ability to breastfeed but also influenced by family support, particularly the involvement of fathers as Breastfeeding Fathers. This community service activity aimed to improve fathers' knowledge, skills, and involvement in supporting exclusive breastfeeding through the Ayah ASI Program, which combined interactive education, practical skill training, and digital mentoring. The activity was conducted in July 2026 in Koto Tinggi Village, the working area of Rambah Public Health Center, Rokan Hulu Regency. A total of 35 participants participated in the program, consisting of 15 married couples, 3 community health cadres, and 2 health workers. The main evaluation involved 15 fathers using a one-group pre-test post-test design. Data were collected through knowledge questionnaires and observation checklists of breastfeeding support skills. The results showed an improvement in fathers' knowledge after the intervention. The proportion of participants with good knowledge increased from 3 respondents (20%) before education to 12 respondents (80%) after education. The skill assessment showed that 12 respondents (80%) achieved a good practice category and 3 respondents (20%) achieved a moderate category in providing breastfeeding support and performing basic skills such as oxytocin massage simulation. In addition, the Ayah ASI Digital community was established as a communication and early mentoring platform after the intervention. The Ayah ASI Program demonstrates that a family-based educational approach can improve fathers' initial capacity to support breastfeeding mothers. However, further evaluation is needed to assess the sustainability of behavioral changes and the long-term contribution of the program to exclusive breastfeeding success.
