Discharge planning is a systematic process to prepare patients for safe and effective care after hospital discharge. However, conventional approaches, such as verbal explanations and printed materials, may limit patients’ access to information. Digital technology, including mobile applications, educational videos, and QR codes, offers opportunities to improve health education, although its implementation remains inconsistent, particularly in Indonesia. To identify scientific evidence regarding the effectiveness of digital education-based discharge planning in improving patient knowledge and attitudes. This literature review followed the PRISMA 2020 guidelines. Articles published between 2021 and 2025 were searched through PubMed, ScienceDirect, and Google Scholar using keywords related to discharge planning, digital health education, patient knowledge, and attitudes. Fifteen eligible articles were included and analyzed narratively. A total of 15 articles met the eligibility criteria and were included in the final synthesis. Digital-based discharge planning positively affected patient knowledge, attitudes, discharge readiness, medication adherence, quality of life, and satisfaction. Most studies used quasi-experimental designs involving mobile applications, educational videos, audiovisual media, and QR codes. However, evidence regarding QR code-based education among minor surgery patients remains limited. Digital health education is a promising strategy to optimize discharge planning and improve patients’ knowledge and attitudes toward self-care after discharge.
