Abstract

Background: The hormonal contraceptive DMPA (Depot Medroxyprogesterone Acetate) is an injectable contraceptive method widely used by women of childbearing age because it is practical and effective in preventing pregnancy. However, prolonged use of DMPA can cause side effects, one of which is secondary amenorrhea. Secondary amenorrhea is the absence of menstruation for three consecutive months or more in women who previously had normal menstruation. The occurrence of secondary amenorrhea often causes anxiety in DMPA users, so research is needed regarding the relationship between DMPA use and the occurrence of secondary amenorrhea. Method: This study employed an analytical design with a cross-sectional approach. A sample of 30 respondents was selected using purposive sampling. The research instruments used were observation sheets and questionnaires. Data analysis used the Chi-Square test with the help of SPSS. Results: The majority of respondents (18 respondents (60.0%) used DMPA hormonal contraception for more than 1 year. Nineteen respondents (63.3%) experienced secondary amenorrhea. The Chi-Square test results showed a p-value of 0.002 (<0.05), indicating a significant relationship between DMPA hormonal contraception use and the incidence of secondary amenorrhea in the Popayato Barat Community Health Center UPTD area. Conclusion: There is a significant relationship between the use of DMPA hormonal contraception and the incidence of secondary amenorrhea in the Popayato Barat Community Health Center UPTD area.

Abstract

Labor pain is one of the most severe pains experienced by maternity mothers and can cause anxiety, fear, and stress that has the potential to slow down the labor process. The Lamaze technique is a non-pharmacological method that emphasizes focused breathing regulation and conscious relaxation to control pain. This study aims to determine the effect of the Lamaze technique on pain intensity and duration during the active phase I in maternity mothers at the Pulubala Gorontalo Health Center. The research design used a pre-experimental one-group pretest-posttest design with a sample of 30 respondents selected through purposive sampling techniques. The research instruments were in the form of a Numerical Rating Scale (NRS) observation sheet and an old observation sheet at period I. Data analysis was carried out using the Wilcoxon test. The results showed a decrease in severe pain from 63% to 10% and an increase in mild pain from 13% to 37%. The length of slow labor decreased from 53% to 10% and there was a rapid labor of 20%. The Wilcoxon test resulted in a p = 0.000 value in both variables, which means that there was a significant difference before and after the intervention. The conclusion of this study is that the Lamaze technique is effective in reducing pain intensity and accelerating the active phase I, so it can be recommended as a non-pharmacological intervention in maternity care.
