Abstract

Aircraft maintenance is an essential aspect of aviation aimed at ensuring airworthiness and keeping components in optimal condition to minimize accidents. The landing gear system supports the aircraft during ground operations, takeoff, and landing, with the Alternate Brake Servo Valve serving as a vital component that regulates hydraulic pressure in the brake system. The failure of Alternate Brake Servo Valve P/N C20374000-2 on Airbus A330 aircraft at PT GMF AeroAsia Tbk has resulted in 57 unscheduled removals between 2020–2024. This study aims to analyze the factors causing these failures, identify the root causes, and provide improvement recommendations. The methods used are Failure Mode and Effect Analysis (FMEA) to identify failure modes and calculate the Risk Priority Number (RPN), and Fault Tree Analysis (FTA) to determine root causes. The analysis shows that Leak From Connection is the dominant failure mode with the highest RPN value (210). The FTA results indicate that the main causes include the absence of specific maintenance procedures, uneven standard practices training, lack of reinspection by Inspectors, non-compliance with manuals, and inaccurate tools used. Based on these findings, several corrective actions are proposed, including developing specific inspection procedures, providing comprehensive standard practices training, implementing a double-check system, emphasizing adherence to the Aircraft Maintenance Manual (AMM), and conducting periodic tool calibration.
