Abstract

Introduction: Premature ovarian insufficiency (POI) is associated with impaired ovarian function and hormonal imbalance, and this meta-analysis evaluates whether cell-based regenerative therapy improves hormonal and follicular outcomes in animal models. Methods: This systematic review and meta-analysis followed PRISMA 2020 guidelines. PubMed and Scopus were searched (December 2025) for in vivo animal studies of POI/POF comparing cell-based regenerative therapy with untreated controls. A random-effects model was used to pool outcomes, reported as mean difference (MD) with 95% confidence intervals (CI). Heterogeneity was assessed using I² statistics. Risk of bias was evaluated using the SYRCLE risk-of-bias tool. Results: Meta-analysis demonstrated that cell-based regenerative therapy significantly reduced follicle-stimulating hormone (FSH) levels (MD -1.79; 95% CI -3.06 to -0.51) and significantly increased estradiol (E2) levels (MD 19.67; 95% CI 17.17 to 22.17) compared with untreated POI controls. In addition, a significant increase in total follicle count was observed in treated groups (MD 31.93; 95% CI 18.08 to 45.78). Considerable heterogeneity was noted in some outcomes, reflecting variability in animal models and intervention protocols. Conclusion: Cell-based regenerative therapy improves hormonal and follicular outcomes in animal models of premature ovarian insufficiency, supporting its potential as a regenerative treatment strategy.
