Abstract

Background: Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental disorder characterized by inattention, hyperactivity, and impulsivity, which can affect a child’s cognitive and executive functions. Aerobic exercise and exergaming are recognized as non-pharmacological interventions. Objective: This study aimed to analyze the effectiveness of aerobic exercise and exergaming in improving cognitive abilities in children with ADHD.Methods: This literature review used the PICO framework and searched through Google Scholar, PubMed, ScienceDirect, and PLOS One (2016-2026). Selection followed PRISMA 2020 guidelines. Randomized Controlled Trials (RCTs) or experimental studies involving children under 18 years of age diagnosed with ADHD were included. Study quality was assessed using the JBI Critical Appraisal Tool. Results: Eight studies were included. JBI scores ranged from 8/13 to 11/13 for RCTs and 6/9 for quasi-experimental studies, indicating moderate to high methodological quality. Aerobic exercise can improve attention, executive function, working memory, and behavioral regulation, while exergaming consistently improved working memory, inhibitory control, and cognitive flexibility. Conclusion: Both aerobic exercise and exergaming may improve cognitive function in children with ADHD. However, current evidence is insufficient to determine whether one intervention is superior because direct comparisons are still limited. Further research with stronger designs and standardized protocols is needed.
