Abstract

Background: Hypertension is a condition where systolic blood pressure is ≥140 mmHg or diastolic blood pressure is ≥90 mmHg, requiring long-term treatment to prevent complications. The success of hypertension therapy is influenced by patient compliance with antihypertensive medication. Purpose: To analyze adherence to antihypertensive medication use among hypertensive patients.. Method: This observational study used a cross-sectional design. The study location was the Bunut Inpatient Community Health Center in Pesawaran Regency. A sample of 130 hypertensive patients was obtained using a purposive sampling technique. Adherence data were obtained using the Morisky Medication Adherence Scale-8 (MMAS-8) questionnaire and analyzed using the Chi-Square test with a 95% confidence level (α=0.05). Results: The study showed that most patients were female (6.7%), aged <65 years (69.2%), had less than a high school education (79.2%), were employed (61.5%), had incomes below the minimum wage (UMP) (76.9%), had uncontrolled blood pressure (80.8%), and had a history of hypertension for >12 months (63.8%). The level of compliance with antihypertensive medication use was low, with 13.1% being compliant and 86.9% being non-compliant. Conclusion: There was a significant association between education (p=0.000), income (p=0.000), blood pressure (p=0.014), and history of hypertension (p=0.037) with adherence to antihypertensive medication use. Meanwhile, gender, age, occupation, belief in treatment, and clarity of drug information were not significantly associated with adherence to antihypertensive medication use. Suggestion: Patients are expected to commit to adhering to their treatment and attending regular health check-ups to ensure therapeutic success. To support this, community health centers (Puskesmas) need to optimize patient education, monitoring, and communicative medication counseling. Meanwhile, future researchers are advised to examine other factors influencing patient adherence to provide a more comprehensive overview.   Keywords: Antihypertensives; Hypertension; Medication Compliance.
