Abstract

Every healthcare facility is required to ensure patient safety in order to improve the quality of care. The 2025 IKP report for East Halmahera shows that only 5 community health centers (31.25%) reported zero IKP incidents, while 11 community health centers (68.75%) did not submit a report. IKP reports tend to increase during Community Health Center accreditation surveys and decrease again after accreditation (IKP application reports). Low incident reporting rates are common, although the levels vary. Development of an Educational Innovation to Implement the “Lapor Pak” Culture (Report Patient Safety Incidents Immediately to the Community Health Center) as a Model for Strengthening the Implementation of Patient Safety Goals at Community Health Centers. The objective of this study is to analyze the educational program for implementing the “Lapor Pak” culture as an effort to improve patient safety incident reporting and foster a patient safety culture. The research design was quasi-experimental with a single-group pretest–posttest approach. The sample consisted of 57 staff members at the Subaim Community Health Center, with simple random sampling used as the sampling technique. The independent variable was education on the implementation of  culture “Lapor Pak” (Report Patient Safety Incidents Immediately to the Community Health Center), while the dependent variables were the rate of Patient Safety Incident (PSI) reporting and patient safety culture. The research instrument was based on the Hospital Survey on Patient Safety Culture Version 2.0 User Guide published by the AHRQ (Agency for Healthcare Research and Quality). Data analysis was performed using a paired t-test. The results of this study show that there was no difference in the number of patient safety incident reports between the periods before and after the implementation of education on patient safety culture (Lapor Pak); p-value = 0.322 (p > 0.05), with the average score increasing from 0.00 to 0.0175, however, this increase was not statistically significant, and patient safety culture improved significantly before and after the implementation of education on patient safety culture (Lapor Pak). p-value = 0.044 (p < 0.05).
