Abstract

Pediatric pneumonia requires accurate medication use because treatment may involve several types of medications and may potentially lead to Drug-Related Problems (DRPs). Medication reconciliation is an important process in pharmaceutical care to ensure the accuracy of patients’ medication information and to help identify drug-related problems during treatment. This study aimed to analyze the relationship between the implementation of medication reconciliation and the occurrence of DRPs in pediatric pneumonia patients admitted to the inpatient ward of RSIA Annisa Blora. This study used an analytical observational design with a cross-sectional approach and retrospective data obtained from patients’ medical records from January to April 2026. The sample consisted of 50 pediatric pneumonia patients who met the study criteria and were selected using total sampling. The implementation of medication reconciliation was assessed based on documentation in the medical records, while DRPs were identified using the Pharmaceutical Care Network Europe (PCNE) version 9.0 classification. The relationship between the implementation of medication reconciliation and the occurrence of DRPs was analyzed using the Pearson Chi-Square test. The results showed that medication reconciliation was performed in 26 patients (52%), while 24 patients (48%) did not receive medication reconciliation. DRPs were identified in 11 patients (22%), while 39 patients (78%) did not experience DRPs. All identified DRPs were classified as category P3.1 according to PCNE version 9.0, namely problems related to the cost-effectiveness of treatment. The analysis showed χ² = 0.037, df = 1, and p-value = 0.848 (p > 0.05), indicating that there was no statistically significant relationship between the implementation of medication reconciliation and the occurrence of DRPs in pediatric pneumonia patients at RSIA Annisa Blora.
