The implementation of Electronic Medical Records (EMR) is an essential part of hospital digital transformation aimed at improving the quality, efficiency, and accountability of healthcare services. The success of EMR implementation is not solely determined by system readiness, but is also strongly influenced by top management support and the readiness of technological infrastructure. This study aims to analyze the role of top management support and technological infrastructure readiness in maintaining the successful use of EMR at Ratu Aji Putri Botung Regional General Hospital, Penajam Paser Utara Regency. This research employed a qualitative approach using a case study method. Data were collected through in-depth interviews, observation, and documentation. The study involved seven informants consisting of hospital leaders, administrative managers, information technology personnel, and healthcare workers as direct users of the EMR system. Data analysis was conducted using descriptive qualitative techniques through data reduction, data display, and conclusion drawing. The results indicate that top management support plays a significant role in maintaining the success of EMR implementation through policy formulation, budget allocation, usage monitoring, as well as training and assistance for users. In addition, the readiness of technological infrastructure has generally supported the use of EMR, although limitations remain in terms of device distribution, network stability, and system development. Overall, the success of EMR implementation at Ratu Aji Putri Botung Regional General Hospital is influenced by the synergy between leadership commitment and sustainable technological infrastructure readiness.
