Abstract

Type 2 diabetes mellitus is a chronic metabolic disease characterized by hyperglycemia and associated with low-grade chronic inflammation. The Neutrophil-to-Lymphocyte Ratio (NLR) is a simple parameter that reflects systemic inflammation, while HbA1c reflects glycemic status over the previous two to three months. This study aimed to determine the relationship between NLR and HbA1c levels among patients with type 2 diabetes mellitus at Bahteramas General Hospital, Southeast Sulawesi Province. This was a quantitative analytic study with a cross-sectional design. The population consisted of 75 patients with type 2 diabetes mellitus during January–March 2026, and 43 respondents were selected using purposive sampling. Data included HbA1c levels, neutrophil counts, and lymphocyte counts; NLR was calculated as the ratio of neutrophil to lymphocyte counts. Univariate and bivariate analyses were performed using Fisher's Exact Test with α = 0.05. The results showed that 39 respondents (90.7%) had increased HbA1c levels (≥5.6%), while 4 respondents (9.3%) were in the normal category (<5.6%). Twenty-seven respondents (62.8%) had low/normal NLR values (<3.0), while 16 respondents (37.2%) had increased NLR values (≥3.0%). Fisher's Exact Test yielded p = 0.279 (p > 0.05). Therefore, there was no statistically significant relationship between HbA1c levels and NLR values among patients with type 2 diabetes mellitus at Bahteramas General Hospital, Southeast Sulawesi Province.
