Abstract

The Emergency Installation (IGD) operates 24 hours and must deliver fast, accurate, and high-quality care, yet increasing patient visits and acuity may cause overcrowding, prolonged waiting times, safety risks, and reduced satisfaction. This study aimed to analyse factors associated with patient waiting time at the Emergency Installation of Ibnu Sina Makassar Hospital Yayasan-UMI, focusing on registration, human resources, and facilities/infrastructure. An analytical descriptive cross-sectional study was conducted in August 2025, with data collected across morning, afternoon, and night shifts through service-flow observation and questionnaires. A total of 90 respondents (estimated population 900) were included using the Slovin formula (10% error) and total sampling based on inclusion criteria. Waiting time was defined as the duration from registration until receiving services or transfer. Data were analysed using univariate statistics and Chi-square tests in SPSS, and ethical approval recommendation was obtained (No: 502/A.1/KEP-UMI/VII/2025). Most respondents experienced fast waiting time (56/90; 62.22%), while 37.78% experienced slow waiting time. Registration was rated fast by 62.22% of respondents, human resources were rated adequate by 72.22%, and facilities/infrastructure by 82.22%. Significant relationships were found between waiting time and registration (p=0.005), human resources (p=0.007), and facilities/infrastructure (p=0.005). The findings suggest that improving registration efficiency, staffing distribution, and facility readiness can reduce waiting time and strengthen service quality. The novelty of this study lies in simultaneously examining these three operational factors in one integrated analysis within this hospital’s IGD, addressing a local research gap.

Abstract

Gout arthritis is an inflammatory disease caused by the deposition of monosodium urate crystals in the joints and surrounding tissues in the setting of hyperuricemia. Demographic and clinical characteristics, kidney function, and treatment patterns need to be identified to describe the profile of patients and support appropriate management. This study aimed to determine the characteristics of patients with gout arthritis at Ibnu Sina YW-UMI Makassar Teaching Hospital in 2025. This was a descriptive retrospective cross-sectional study using secondary data obtained from medical records. All patients with gout arthritis in 2025 who met the inclusion criteria were included using total sampling, resulting in 25 patients. Data were analyzed descriptively and presented as frequency distributions. Most patients were male (84%), and the largest age group was 46–55 years (36%). Regarding body mass index, 56% of patients were in the normal category, while 44% were classified as overweight or obese. Recorded anti-inflammatory therapies included colchicine (76%), nonsteroidal anti-inflammatory drugs (NSAIDs) (56%), and prednisone (24%), while the urate-lowering therapy allopurinol was recorded in 80% of patients. Combination therapy was received by 80% of patients, with the combination of NSAIDs, colchicine, and allopurinol being the most frequently recorded regimen (30%). Tophus was found in 40% of patients, while a history of recurrent attacks was recorded in 28%. Elevated creatinine levels were found in 61.9% of male patients, whereas all female patients had normal creatinine levels. This study demonstrates that patients with gout arthritis at Ibnu Sina YW-UMI Makassar Teaching Hospital in 2025 were predominantly male, with variations in body mass index, tophus, recurrent attacks, kidney function, and treatment patterns.

Abstract

Postoperative pain following gynecological laparotomy is a clinical problem requiring optimal analgesic management. Intravenous ibuprofen and paracetamol can be used as components of multimodal analgesia; however, comparative evidence regarding their effects on pain intensity and inflammatory response in patients undergoing gynecological laparotomy remains limited. This study aimed to compare postoperative pain intensity and changes in the Neutrophil-to-Lymphocyte Ratio (NLR) in patients undergoing gynecological laparotomy who received preemptive intravenous ibuprofen or paracetamol. This experimental pre-post test study included 32 patients divided into ibuprofen (n=16) and paracetamol (n=16) groups. Pain intensity was assessed using the Numeric Rating Scale (NRS) before surgery and at 2, 6, 12, and 24 hours postoperatively. NLR was assessed preoperatively and 24 hours postoperatively. Between-group comparisons were performed using statistical tests according to data distribution. No statistically significant differences in NRS scores were found between the groups at the preoperative, 2-, 6-, 12-, or 24-hour assessments (p>0.05). At 6 hours postoperatively, the mean NRS score was lower in the ibuprofen group than in the paracetamol group, but the difference was not statistically significant (2.88±0.62 vs. 3.31±0.79; p=0.102). The mean change in NLR also did not differ significantly between the paracetamol and ibuprofen groups (8.60±16.91 vs. 8.79±15.08; p=0.836). Thus, this study found no statistically significant differences between preemptive intravenous ibuprofen and paracetamol in postoperative pain intensity or changes in NLR following gynecological laparotomy.
