Abstract

General Background: Health clinics require accurate and efficient information management to support organized administrative and medical record services. Specific Background: At Afifa Prambon Clinic, patient registration and medical record reporting were still conducted manually using Microsoft Excel, creating risks of duplicate medical record numbers, data management errors, and limited data integration. Knowledge Gap: The existing process lacked an integrated information system that could support patient registration, visit recapitulation, and medical record reporting while providing differentiated user access. Aims: This study aimed to analyze requirements and design a web-based outpatient registration and reporting information system for Afifa Prambon Clinic. Results: The study used primary data obtained through interviews, direct observation, and literature review, with the Waterfall method applied to the system development process. The resulting design provides patient registration, visit recapitulation, medical record reporting, user login, differentiated access, and data export functions. The registration form accommodates patient identification and demographic information, including medical record number, national identification number, name, date of birth, sex, address, education, occupation, religion, telephone number, and status. Novelty: The study integrates outpatient registration and reporting functions into a web-based application designed according to the operational requirements of Afifa Prambon Clinic. Implications: The proposed system is expected to facilitate patient registration, support visit reporting, reduce the risk of data errors and duplication, and improve administrative service management at the clinic. Key Findings The study identified three main user roles with different access responsibilities. The registration form accommodates comprehensive patient identification and demographic data. The design provides visit recapitulation, printing functions, reporting, and routine data backup.
