Abstract

This study aimed to describe registration officers’ perceptions of electronic medical record (EMR) implementation, reported operational barriers, and feature-development needs in the registration units of community health centers (Puskesmas) in Surabaya. A descriptive cross-sectional study was conducted in July 2023. Nine medical record and health information officers were recruited through incidental sampling, with one officer from each of nine different Puskesmas. Data were collected using a 24-item questionnaire covering system quality, information quality, service quality, use, user satisfaction, and perceived net benefit and were analyzed using frequencies and percentages. All respondents rated the six dimensions as good or very good. Information quality, use, and perceived net benefit were each rated very good by six respondents (66.7%), whereas user satisfaction was rated good by eight respondents (88.9%). All respondents reported slow or unstable internet connectivity, patients arriving without an identity card by five (55.6%), and power outages by four (44.4%). Integration of general consent into the registration module was the most frequently requested feature, reported by five respondents (55.6%). These findings indicate generally favorable perceptions across nine institutional settings, while the reported connectivity barriers and feature needs may inform managerial assessment of infrastructure reliability and workflow fit. However, the findings do not constitute an objective assessment of institutional performance or establish causal relationships. They should be interpreted as an initial cross-institutional description based on one respondent from each Puskesmas.
