Abstract

The mechanism through which self-blame operates within sexual relationships among individuals with a history of childhood maltreatment who develop disorganized schizophrenia remains poorly understood. This study aimed to explore how such misattribution operates within the psychological dynamics of an individual with a history of childhood maltreatment who developed disorganized schizophrenia. A qualitative approach with a single case study design was employed. The participant was a 48-year-old male with the initials F.C., diagnosed with F20.13 Hebephrenic Schizophrenia Episodic Remittent, undergoing inpatient treatment at Hospital X. Data were collected through systematic observation, semi-structured interviews, alloanamnesis, and medical record review, then analyzed using thematic analysis. Four major themes were identified: childhood maltreatment as the foundation of vulnerability, the formation of negative self-representation, sexual relationships and self-blame misattribution, and the accumulation of psychological pressure toward disorganized schizophrenia. Findings indicate that physical and emotional violence from the caregiver figure formed disorganized attachment, negative self-schema, and characterological self-blame, which manifested most concretely when the participant constructed a narrative of biological inadequacy to attribute his partner’s departure. Within the diathesis-stress framework, this misattribution served as the convergence point between childhood vulnerability and adult life stressors that accelerated the clinical trajectory toward disorganized schizophrenia.
