Abstract

Breast cancer ranks second in global cancer incidence among women and is the leading cancer type and cause of cancer-related mortality among women in Indonesia. Four molecular subtypes Luminal A, Luminal B, HER2-enriched, and triple-negative breast cancer (TNBC) are identified through immunohistochemistry (IHC) and differ in clinical behavior and prognosis, but their relationship with histopathological features remains unexamined in this region. This study evaluated associations between IHC-defined molecular subtypes and histopathological type and grade among breast cancer patients at NTB Provincial Hospital. This cross-sectional study used medical records from 361 consecutive patients who underwent IHC testing in 2023-2024. Subtypes were compared using Pearson’s chi-square test, with Cramér’s V measuring effect size; luminal and non-luminal groups were also analyzed separately. Luminal B was most common (47.1%), followed by HER2-enriched (18.8%), TNBC (18.0%), and Luminal A (16.1%). Ductal carcinoma accounted for 85.6% of cases, and 72.3% were Grade 3. Histological grade differed significantly across subtypes (p=0.019; V=0.17), with the lowest proportion of high-grade tumors in Luminal A and the highest in TNBC. Histological type did not differ significantly (p=0.063; V=0.14). Luminal versus non-luminal status was associated with histological type (p=0.026), but not grade (p=0.154).
