Abstract

General Consent is an important document that serves as evidence of the patient's consent to medical and administrative services during treatment. At Sumber Hurip Hospital, monitoring the completeness of General Consent forms is still carried out manually using a spreadsheet, making the process less effective and prone to delays and errors. This study used a qualitative method with a descriptive approach through observation, interviews, and document review. The study aimed to design a monitoring system for the completeness of General Consent for inpatients using the Agile method, which consists of Requirements, Design, Development, Testing, and Launching. The system was designed using a Flowmap, Context Diagram, Data Flow Diagram (DFD), Entity Relationship Diagram (ERD), and user interface design. The resulting system is designed to display General Consent completion status, dashboard, patient data, and monitoring reports. This system is expected to improve staff efficiency and facilitate monitoring of General Consent completeness.
