Abstract

Coronary heart disease constituted one of the primary causes of global morbidity and mortality, with hypertension serving as a major risk factor that accelerated disease progression. Management of this chronic condition requires a holistic, comprehensive, and patient-centered family medicine approach to achieve optimal risk factor control. This study utilized a case report design by applying a family medicine approach to a 44-year-old female patient residing in the Krendang area, West Jakarta. Data collection was conducted directly through home visits, clinical interviews, physical examinations, and environmental observations. The patient worked as an opak (traditional rice cracker) seller, presenting with a chief complaint of radiating left-sided chest pain and shortness of breath during physical activity. The patient had a history of coronary heart disease post-stent placement two years ago, hypertension, hypercholesterolemia, and a family history of cardiovascular disease. Physical examination demonstrated a blood pressure of 158/101 mmHg, which was classified as stage II hypertension, a respiratory rate of 24 breaths per minute, and obesity with a body mass index of 32.36 kg/m². The patient's primary challenge was therapeutic non-compliance in the form of repeated medication discontinuation due to economic constraints, with a monthly income of 600,000 rupiahs, which was further exacerbated by the habit of consuming oily foods, a lack of physical activity, and crowded housing conditions. In conclusion, a family medicine approach that considers biopsychosocial aspects is highly critical to overcome economic barriers, optimize pharmacological therapy, and sustainably improve the patient's quality of life.
