Abstract

The implementation of Hospital Management Information Systems (HMIS) has become a key component of healthcare digital transformation. However, previous studies have predominantly employed quantitative approaches, focused on private or referral hospitals in large cities, and provided limited understanding of users' lived experiences in regional public hospitals. This study aimed to evaluate the utilization of the Hospital Management Information System (HMIS) in supporting employee work effectiveness and to formulate an ideal HMIS implementation model at the Regional General Hospital of Tabanan Regency, Indonesia. A qualitative phenomenological approach was employed using the HOT-Fit Framework (Human–Organization–Technology–Fit) as the analytical framework. The research object was the utilization of HMIS in supporting employee work effectiveness, while the research subjects consisted of 13 purposively selected participants, including the hospital director, HMIS administrator, physicians, nurses, and administrative and medical records staff. Data were collected through in-depth interviews, non-participant observation, and document analysis, and were analyzed using thematic analysis. The findings indicate that HMIS has been intensively utilized and has improved work effectiveness by accelerating information access, strengthening interdepartmental coordination, increasing administrative efficiency, and supporting decision-making processes. Nevertheless, several challenges remain, including inadequate infrastructure, digital literacy gaps, double documentation practices, and the absence of continuous training programs. This study proposes an ideal HMIS implementation model comprising seven key components as a strategic recommendation for optimizing system implementation. It is concluded that the effectiveness of HMIS utilization depends on the alignment of human, organizational, and technological dimensions, requiring integrated improvements to enhance employee performance and hospital service quality.
