Student information at SMP Negeri 4 Medan is scattered among subject teachers, homeroom teachers, guidance and counseling (BK) teachers, activity coaches, and parents, with no single medium connecting it, so records are often missed. This study aimed to develop SIM-PD Mini as an integrated medium for recording and monitoring students' discipline, personal-social conditions, and interests and talents. The study used the ADDIE development model with a case study approach. Data for the analysis stage were obtained through interviews with one homeroom teacher and one BK teacher, along with documentation. The evaluation used interviews, observation, and a Likert-scale questionnaire completed by 15 teachers. The analysis showed that student information was not yet integrated, violations were not recorded routinely, no safe channel for reporting bullying was available, and mapping of students' interests and talents was not yet comprehensive. Based on these findings, SIM-PD Mini was developed as a web-based education dashboard built with Canva and accessible at any time. The dashboard contains modules aligned with the seven pillars of student management, plus a Suggestion Box module and a BK Dashboard, with access restricted by user role and an explicit notice on the confidentiality of student data. The questionnaire yielded an overall score of 90.4%, categorized as very helpful, and teachers found the dashboard easy to use and felt supported because records were no longer forgotten. The Suggestion Box module also worked as a channel for students to submit suggestions. This study produced SIM-PD Mini, which has potential for further development to integrate student information and make follow-up easier for teachers, BK teachers, and parents.
