Abstract

Pain is a defence mechanism for the body that arises when the tissues in the body are being damaged. Pain therapy can be used in the form of NSAIDs. In prescribing NSAIDs, there is a risk of drug-related problems in the form of treatment duplication without gastroprotective. This study aims to determine the description of the type of duplication of NSAIDs without gastroprotective and explain pharmacist intervention efforts in reducing drug-related problems in prescribing NSAIDs. This retrospective observational study is conducted on all incoming prescriptions at the pharmacy satellite in March-June 2022 at one of the hospitals in Indonesia. The results of this study found 53 cases of duplication of NSAID therapy without gastroprotective from a total of 3,200 prescriptions, with the most duplication of NSAID therapy between Ibuprofen 400 mg and Mefenamic Acid 500 mg 24 cases. Pharmacist intervention in the form of clinical prescription screening is beneficial to avoid the risk of drug-related problems in prescribing NSAIDs.

Abstract

The use of dual NSAIDs without gastroprotective is one of the drug related problems that often occurs in hospitals, the purpose of giving dual NSAIDs is to increase the effectiveness of pain relief in reducing pain in patients. However, the use of dual NSAIDs without gastroprotective increases the risk of side effects on the gastrointestinal tract, kidneys, and cardiovascular system. This study aimed to examine the risk factors for duplication of NSAID therapy without gastroprotective in the outpatient pharmacy unit at a type C hospital in Banyuasin district. This analytical study used a cross-sectional research design to determine the influence of risk factors on the incidence of duplication of NSAIDs therapy without gastroprotective in the outpatient polyclinic. Data were analyzed using Pearson Chi Square analysis. The results of the bivariate analysis showed that older age, polypharmacy, and multipathology were factors associated with the incidence of NSAID duplication in outpatients with p values (OR) of 0.047 (1.101), 0.025 (2.63), 0.025 (2.63), and 0.025 (2.63), respectively. Abstract

Polypharmacy is one of the risk factors for potential drug interactions that can cause unwanted drug reactions and can also increase the risk to pediatric patients. This study aims to see if there is a relationship between polypharmacy and the incidence of drug interactions in pediatric inpatient prescriptions that will be carried out at Embung Fatimah Regional General Hospital, Batam City. This study uses a cross-sectional method, an observational research design conducted to determine the relationship between polypharmacy as the independent variable and the incidence of drug interactions as the dependent variable, with measurements taken at a single point in time. The target population to be studied is the total number of inpatient pediatric prescriptions at Embung Fatimah Hospital as many as 440 prescriptions. The sample used in this study was taken from the affordable population with inclusion and exclusion criteria. Polypharmacy prescriptions for pediatric inpatients at Embung Fatimah Regional General Hospital (RSUD) Batam City based on the results of research from a total of 168 prescriptions there were 123 (73.2%) polypharmacy prescriptions and 45 non-polypharmacy prescriptions. Drug interactions in pediatric inpatient prescriptions at the Regional General Hospital (RSUD) Embung Fatimah Batam City based on the results of the study from a total of 168 prescriptions there were 44 prescriptions experiencing drug interactions and 124 did not experience drug interactions. There is a significant relationship between polypharmacy and the incidence of drug interactions in pediatric inpatients at Embung Fatimah Regional General Hospital (RSUD) Batam City. The Chi-Square test resulted in a P-value ≤ 0.05, indicating that the null hypothesis (H₀), which states that there is no relationship between polypharmacy and drug interactions, is rejected. Consequently, the alternative hypothesis (H₁), which states that polypharmacy is significantly associated with the incidence of drug interactions, is accepted.

Abstract

Epilepsy is a condition of the onset of attacks in the form of abnormal, irregular brain nerve cell movements, occurring repeatedly and causing temporary motor, sensory or mental function disorders. The treatment of epilepsy has costs that must be known, including direct medical costs, indirect medical costs, and indirect costs; this study aims to determine the cost of illness of epilepsy in one hospital in banyuasin regency with retrospective costing in outpatients. The research method used in this study is a cross-sectional study using patient medical record data and cost data taken from BPJS claims data and hospital rate patterns. This study uses the BPJS perspective and societal perspective, the cost data taken from the BPJS perspective is the result of BPJS claims according to the INACBGs. Package tariff and for the socital perspective the data taken is doctor consultation data, administrative costs, laboratory costs, drug costs and transportation costs and lost productivity costs carried out by interview method for 12 months. The results of this study indicate that the cost of epilepsy outpatients from a societal perspective is Rp.3,141,414 and for BPJS perspective is Rp.2,338,567.
