Abstract

Penderita diabetes mellitus tipe 2 mengalami perubahan fisik, psikologis, dan seksual. Perubahan ini dapat menyebabkan timbulnya masalah, ketidakstabilan kadar gula dalam darah. Tujuan dari penelitian ini adalah untuk membuktikan pengaruh Acceptance and Commitment Therapy (ACT) dalam pengendalian kadar gula darah pada penderita DM tipe 2. Penelitian ini adalah penelitian quasy eksperiment dengan menggunakan metode pretest posttest with control group design. Responden dalam penelitian ini adalah penderita DM tipe 2 sebanyak 50 orang dan diambil dengan teknik simple random sampling dan dibagi menjadi dua kelompok yaitu kelompok kontrol dan kelompok perlakuan. Data dikumpulkan dengan menggunakan lembar observasi hasil pengukuran BS stick. ACT diberikan pada kelompok perlakuan satu minggu sekali selama empat minggu. Data dianalisis menggunakan uji statistik yaitu Paired t-Test dan Independent t-Test, dan regresi linear berganda dengan signifikansi p<0,05. Hasil analisis menunjukkan bahwa terjadi perbedaan kadar gula darah yang signifikan antara kelompok kontrol dan kelompok perlakuan dengan p-value = 0,000. Intervensi ACT adalah variabel yang paling berpengaruh untuk kadar gula darah penderita DM tipe 2 dengan sig 0,000. Intervensi ACT efektif terhadap pengendalian kadar gula darah penderita DM tipe 2. ACT dapat dilakukan dengan baik karena responden dan terapis proaktif, dan akan lebih baik melibatkan peran dan dukungan keluarga untuk penelitian selanjutnya. Kata Kunci: Acceptance and Commitment Therapy (ACT), penderita DM tipe 2, kadar gula darah.

Abstract

Rutinitas olahraga dan rekreasi pada ibu hamil banyak terganggu oleh pandemi, terutama di daerah metropolitan Kegiatan rekreasi bertujuan mempererat ikatan ibu dan janin sebelum kelahiran, serta liburan saat hamil juga membantu menghalau stres yang rentan menyerang. Waktu yang paling tepat saat melakukan liburan atau rekreasi yaitu ketika usia kehamilan memasuki trimester kedua, atau ketika usia kandungan antara 20-30 minggu dan disarankan ibu hamil pada waktu melakukan perjalanan setidaknya 2 jam sekali berhenti untuk mengurangi tekanan. Tujuan kegiatan ini adalah Meningkatkan pengetahuan dan keterampilan ibu hamil dan masyarakat tentang pentingnya olahraga dan rekreasi di masa kehamilan. Metode yang diterapkan dalam pengabdian masyarakat ini adalah model edukasi dan memberikan keterampilan kepada kader, ibu hamil dan keluarga dan setelah satu bulan dievaluasi hasil pre dan post pemberian edukasi. Hasil analisa data dengan menggunakan uji statistik Wilcoxon Signed Rank Test didapatkan hasil p value < 0,05 yang berarti ada perbedaan pengetahuan dan keterampilan pre dan post pemberian edukasi dan keterampilan. Hasil dari kegiatan didapatkan bahwa pengetahuan dan keterampilan tentang pentingnya olahraga dan rekreasi di masa kehamilan ibu hamil, keluarga dan kader sebelum diberikan intervensi 50 % kurang dan 50 % cukup. Hasil sesudah pemberian edukasi dan keterampilan didapatkan hasil 25% cukup dan 75 % baik. Pemberian edukasi dan keterampilan tentang tentang pentingnya olahraga dan rekreasi di masa kehamilan bermanfaat meningkatkan perilaku sehat dalam merawat kehamilan. Pengetahuan merupakan faktor utama dalam merubah perilaku untuk mencapai derajat kesehatan ibu hamil yang optimal dan hal ini sesuai dengan konsep dari Theory Planned of Behavior. Kesimpulan dalam kegiatan ini adalah pemberian edukasi dan keterampilan dapat mempengaruhi perilaku sehat yaitu ibu hamil dan keluarga mampu merawat dirinya selama kehamilan. Keterlibatan keluarga dan kader kesehatan sangat berpengaruh dalam mewujudkan perilaku tersebut dengan cara berkolaborasi dengan baik dengan tim kesehatan.

Abstract

Introduction: Stroke treatment must be done quickly and precisely because the longer the stroke is not treated immediately, the higher the severity of the stroke. One of the complications of stroke is cerebral circulation disorders which cause several symptoms including cerebral tissue hypoxia. Objective: This study aimed to analyze nursing care for stroke patients by giving elevated head positions to overcome the nursing problem risk of ineffective cerebral perfusion at RSI Surabaya A. Yani. Methods: This research design is a case study with  Mrs. M who has nursing problems at risk of ineffective cerebral perfusion. Data collection was done directly by interview, observation, and physical examination of Mrs. M and her family directly. Nursing interventions for providing elevated head positions are carried out for three days for 30 minutes daily. Results: The results showed that after implementation there was a significant increase in oxygen saturation in Mrs. M on the first day SpO2 95%, on the second day SpO2 97%, and on the third day SpO2 99%. In addition to an increase in oxygen saturation, there was an increase in the outcome criteria to be achieved, namely the level of consciousness on a scale of 4 (moderately improved), headache on a scale of 4 (moderately decreased), average blood pressure values on a scale of 4 (moderately improved), and awareness on a scale of 4 (moderately improved). Conclusions: Giving an elevated head position is effective in increasing oxygen saturation in stroke patients with nursing problems of ineffective cerebral perfusion risk. Room nurses can perform elevation head positioning to improve oxygen saturation in stroke patients with nursing problems at risk of ineffective cerebral perfusion.

Abstract

Background: Hypertension sufferers who rarely exercise, smoke, consume alcohol, are overweight, cannot manage stress, often stay up late, do not comply with taking medication and eat whatever they want without paying attention to salt intake can cause uncontrolled blood pressure which can lead to hypertension complications and even death. This behavior is an ineffective health management caused by lack of exposure to information related to hypertension and ineffective health care patterns in the family. Objective: The aim of this research is to analyze family nursing care by applying brisk walking exercise to hypertensive clients to overcome ineffective health management in the Tandes Subdistrict, Surabaya. Methods: The research design used a case method with the subject used being 1 patient with ineffective health management nursing problems. The research was conducted in the Tandes area for 2 weeks with data collection methods including assessment. diagnosis, intervention, action and evaluation. Result: The results of case studies on hypertensive clients with ineffective health management nursing problems show that after being given nursing care therapy by implementing 5 family tasks using a modified brisk walking exercise, the results of improved health management were obtained, which was indicated by the client and family understanding more about hypertension (understanding, risk factors, signs of symptoms and diet), the client's body looks fitter, the client's blood pressure which was initially 170/100 mmHg at the time of the assessment gradually dropped to 140/90 mmHg, the client began to pay attention to diet, the family was willing to help with treatment and took the client for control to a health facility nearest. Conclusion: Evidence based nursing implementation of brisk walking exercise which is carried out with a training frequency of 3 times a week, for 15-30 minutes with an average walking speed of 4-6 km/hour is shown to be effective in reducing ineffective health management.

Abstract

Background: In tuberculosis, excessive secretion production can cause airway obstruction, resulting in the problem of ineffective airway clearance. One of the nursing interventions that can be applied to clear sputum in the airways is chest physiotherapy and effective coughing techniques. Effective cough techniques and chest physiotherapy to make things easier expelling sputum so that the breathing process can run well so that it meets the body's oxygen needs. Objective: The aim of the research was to determine the application of effective coughing and chest physiotherapy to reduce the ineffectiveness of airway clearance in tuberculosis clients carried out independently. Methods: This research method uses a descriptive method with a case study approach. The instrument used is an observation sheet. The number of respondents included one tuberculosis family with a nursing diagnosis of ineffective airway clearance. Result: The results of applying effective cough techniques and chest physiotherapy given twice per day for three days with a duration of 10 to 15 minutes for each appointment showed that complaints of coughing and shortness of breath were reduced and it was easier to expel phlegm. Conclusion: The application of effective coughing techniques and chest physiotherapy has proven effective in reducing ineffective airway clearance in tuberculosis clients. Effective cough techniques and chest physiotherapy can be used as alternative measures to overcome ineffective airway clearance.

Abstract

Introduction: Auditory hallucinations are a common sensory perception disorder affecting approximately 70% of individuals diagnosed with schizophrenia. Various factors contribute to hallucinations, including physical, emotional, intellectual, social, and spiritual dimensions. Objective: This study aims to evaluate the effectiveness of music therapy in mitigating sensory perception disorders in individuals with schizophrenia. Methods: A descriptive case study approach was employed. Data were collected through interviews, observations, and physical examinations involving clients and their families. The Standard Operating Procedure (SOP) for Classical Music Therapy was utilized. The study focused on a single individual experiencing auditory hallucinations within a family setting. Results: Implementing music therapy over four days (one session per day, approximately 13 minutes) resulted in reduced auditory disturbances, decreased anxiety, improved sleep quality, and a decline in the Auditory Hallucination Rating Scale (AHRS) score from 24 to 11, along with a reduction in the Hamilton Rating Scale for Anxiety (HRS-A) score from 26 to 12. Conclusions: Classical music therapy effectively reduces auditory hallucinations and alleviates anxiety in individuals with schizophrenia. Therefore, it is recommended as a non-pharmacological intervention and an independent nursing practice for managing sensory perception disorders.

Abstract

Background: Pulmonary Tuberculosis is one of the top ten causes of death worldwide. Case detection rate in East Java ranks eighth in Indonesia, namely 42.922 cases with treatment coverage of 44.7% of the treatment coverage target set at a minimum of 80%. Factors related to success include knowledge, attitudes, beliefs, work, etc. One of the supporting factors related to the success of treatment is the role of the drug swallowing supervisor. Purpose: The purpose of this study was to determine the relationship between the role of drug swallowing supervisor in the treatment of patients with pulmonary tuberculosis. Methods: The research design uses correlational, population analytic methods. Respondents used are 41 out of 46 pulmonary tuberculosis sufferers at health center in Surabaya using simple random sampling technique. The independent variable of the study is the role of drug swallowing supervisor and the dependent is the success of the treatment. The instrument use questionnaire and medical records. Results: Data processed using chi square test analysis, value = 0.05. The results showed that almost all of the 41 respondents (87.8%) had supportive drug swallowing supervisor, almost all (78%) the treatment was successful. Based on statistical tests obtained (p = 0.000) which proves that the role of drug swallowing supervisor has a relationship with the success of treatment in patients with pulmonary tuberculosis. Conclusion: The greater the supporting role, the greater the success rate of pulmonary tuberculosis treatment. It is expected that drug swallowing supervisor can motivate, assist, remind, and participate in socialization activities for handling pulmonary tuberculosis patients.
