Abstract

Indonesia is aiming for measles elimination status by 2020. However, high number of measles cases are still prevalent and there are still coverage differences among provinces. Measles immunization coverage also varies among surveys and routine coverage report. With the addition of second dose measles vaccination (MCV2), measles infections in the country is expected to decrease. However, MCV2 coverage has been low after 2 years of implementation. Aceh Province was chosen for this study because its low coverage. This study aimed to measure the coverage and investigate the determinants of MCV2 in Aceh Jaya, Aceh Province. Dataset from “Assessment of the New 2nd Dose Measles Policy and the School-based Immunization Program in 2 Provinces (Aceh and South Sulawesi)” was used. There were a total of 300 children aged 25 – 37 months with coverage of MCV2 54% in Aceh Jaya District, Aceh Province. After further selection, 129 observations underwent bivariate and multivariate analysis using logistic regression. After controlling all variables, only experience of prompt service at the healthcare provider was associated with child receiving MCV2. We suggest future studies to look more into health care services and willingness to get vaccination. Additionally, with this result we hope the government could improve health care services in their facilities in order to achieve higher coverage.

Abstract

Background: Premarital sex is culturally unacceptable in Indonesia and education on safe sex practice remains controversial. Meanwhile, Indonesia Demographic and Health Surveys (IDHS) show gradual increase in the prevalence of sexually-active adolescents nationwide, particularly among unmarried males. Unfortunately, condom use is low among this population and it is unclear whether it relates to inadequate knowledge on safe sex practice including condoms. Objective: to see whether there is an association between knowledge on condom functions and condom use among adolescents.Method: cross-sectional study of 913 Indonesian unmarried males aged 15 – 24 who have had sex (IDHS Adolescent Reproductive Health 2012 dataset). The independent variable is knowledge on condom functions while the dependent variable is the use of condoms. Statistical analysis is performed using Chi Square and Cox regression.Result: The prevalence of condom use is about twice higher in respondents with sufficient knowledge on condom functions (31%), than in respondents without (15.1%); adjusted PR 2.38 (95%CI 1.47 3.85). Conclusion: Having knowledge about condoms is positively associated with safer sex practice among sexually active adolescents. Banning information on condoms may place sexually-active adolescents into unprotected sex. Education on safe sex practice is needed but should be cautiously tailored to meet cultural values

Abstract

Hypertension is a common comorbidity in patients with type 2 diabetes mellitus (T2DM) and substantially increases the risk of both macrovascular and microvascular complications. Data on the determinants of hypertension status among T2DM patients in primary healthcare settings remain limited. This study aimed to analyze the determinants of hypertension status in T2DM patients at Puskesmas Menteng during 2024–2025. This was an analytic observational study employing a cross-sectional design. Study subjects comprised T2DM patients registered and undergoing examinations at Puskesmas Menteng over the 2024– 2025 period. Data were extracted from electronic medical records, included age, sex, occupation, socioeconomic status, blood pressure, hypertension status, diabetes duration, body mass index (BMI), kidney function (estimated glomerular filtration rate; eGFR), total cholesterol, low-density lipoprotein cholesterol, triglycerides, and urinary albumin. Hypertension status was classified as hypertensive or non-hypertensive. Multivariable logistic regression analysis was performed, and the results were reported as adjusted odds ratios (aORs) with 95% confidence intervals (CIs). A total of 462 patients with T2DM were analyzed, including 349 patients (75.5%) with hypertension and 113 patients without hypertension. In the multivariable analysis, obesity was significantly associated with increased odds of hypertension (aOR 2.36; 95% CI:1.17–4.78;p=0.017). Microalbuminuria (aOR 1.82;95%CI:1.11–3.00;p=0.019) and macroalbuminuria (aOR 3.47;95%CI:1.29–9.32;p=0.014) were also significantly associated with hypertension. HbA1c levels <7% were positively associated with hypertension status, whereas associations with eGFR categories should be interpreted with caution due to limited sample size and wide confidence intervals in advanced kidney function categories. These findings highlight the importance of weight control, monitoring kidney function monitoring, and early detection of albuminuria in preventing and controlling hypertension among patients with type 2 diabetes mellitus in primary healthcare settings.Keywords: Type 2 diabetes mellitus, Hypertension,  Glycemic Control, Hemoglobin  A1c, Primary Health Care
