Abstract

This study aimed to determine medical action approval form completeness (informed consent) information system design using Microsoft Visual Studio 2010 usage in Rumah Sakit Al Islam Bandung. The research used qualitative method with descriptive approach. While data collection techniques used by direct observations, interviews and completed with a literate review that had a relationship to the subject matter. The development method used was Waterfall. From making this information system the writer used programming language Microsoft Visual Studio 2010 and storage of data using Microsoft Access 2007. From the results conducted there were some problems found, such as:lack in socialization of standard operating procedure for informed consent making, many informed consent forms are still not filled in completely, this is unfortunate given the importance of legal aspects contained in informed concent and, proses of analyzing for completeness informed consent form are still uses Microsoft Excel, so that analysis time becomes longer. The suggestions were given by: (1). Socializing the standard operating procedure of making an informed consent to the relevant officers;(2).Developinga new information system for the data processing analysis the completeness of informed consentfilling for producing reports more accurate and time efficiency in the analysis process.

Abstract

This research aimed for the inpatient medical resume information system design completeness at RSUD Meuraxa Kota Banda Aceh. Writer did the research observation at RSUD Meuraxa Kota Banda Aceh. Method research were used qualitative methode with descriptive approach, Development methode used waterfall development method. From result of this research couducted found problems: checking process of medical resume s was not effective so that completeness system of medical resume s did not run properly, there was a the inpatient medical resume was not complete, there was often implementation has been delayed return inpatient medical record. Therefore, the authors make the design of information system medical resume inpatients Unified Modeling Language (UML) as a design method, using Hypertext Processor (PHP)as a program application and MySQL as database.The suggestions from writer is there should have been the development of medical resume information system design completeness to ease process of the required information, having a sosialization to all health officer about filling medical resume s, need to remind to an officer of the room on must of filling medical resume s

It conducts an assessment of the literature while for the description of the information system using an object-oriented depiction method with UML tools. The waterfall is used as a software development method. The PHP programming language is a programming language and database Mysql. Based tests on academic information systems at practitioner polytechnics can be used properly and effectively, making it easier for users to carry out academic administrative processing.

Abstract

This research aims to design a student attendance information system. In developing the information system, we used the method of observing research objects and conducting literature studies. We used object-oriented modelling with UML tools to describe the student attendance information system. In addition, extreme programming was used to develop the student attendance information system. In creating a student attendance information system using the PHP and MySQL programming languages as the System Management Database, the test results of the student attendance information system at the practitioner Polytechnic can be used well and effectively.
