Abstract

Adherence is one of the important things for diabetic patients to control their blood glucose level and preventing for complications in the future. There are two major factors that influence the adherence, patientâ??s knowledge and motivation. Design of this study was sequential mixed method. Thirty type 2 diabetes mellitus patients were recruited based on patient medication record prepared by the community pharmacist. At the first stage, subjects were asked to answer four validated questionnaires that related with level of knowledge (health literacy and medication knowledge) and motivation (readiness to change and social support). At the second stage, they were interviewed based on their response to readiness to change questionnaire. All data collected were then analysed descriptively and qualitatively. From knowledge aspects, there were 93% of patients has high level of health literacy and 87% has high medication knowledge. From motivation aspects, there were 70% has changed and maintained their adherence, 20% were still prepare to change (preparation) and 10% were not ready to change (contemplation). Need for social supports were very high (97%) within the diabetic patients. Most of the subjects (80%) have high level of knowledge and motivation. There were 17% who has variable knowledge and motivation and only 3% who has low level of knowledge and motivation.

Abstract

Hipertensi merupakan salah satu penyakit utama yang menyebabkan kematian di dunia. Riskesdas 2018 menunjukkan prevalensi hipertensi sebesar 34,11% pada penduduk berusia >18 tahun di Indonesia. Komplikasi hipertensi berdampak pada pembuluh darah, jantung, otak, ginjal, mata, dan organ lainnya. Pasien dengan hipertensi tidak terkontrol dan menjalani operasi dapat mengalamipenurunan tekanan darah yang besar selama pemberian anestesi yang menyebabkan kerusakan organ. Laporan ini bertujuan mendiskusikan kasus seorang pria berusia 67 tahun dengan Batu Renal Staghorn disertai hipertensi yang menjalani operasi. Dokter layanan primer berperan dalam mendiagnosis pasien hipertensi, mengupayakan tekanan darah pasien tetap terkontrol, mengoptimalkan kondisi pasien pada persiapan perioperatif guna menurunkan kemungkinan timbulnya komplikasi akibat hipertensi.

Abstract

Introduction: Patients with advanced-stage cancer should be able to stay at home with their families without suffering from pain. This case report aims to discuss challenges in pain management of a patient with bone metastasis of prostate cancer who received epidural analgesia at home. Challenges were related to the patients, health professionals, and system. Understanding each factor will bring insights to develop a workable pain management program in the palliative setting. Case Presentation: A 66-year-old male was diagnosed with left sacroiliac joint pain due to metastatic prostate cancer. After a three-year history of prostate cancer and ineffective chemotherapy, the patient complained of pain in the left hip that did not improve with conventional medications (paracetamol, tramadol, and Morphine Sustained Release/MST). In response, the consideration of an epidural implant emerged as a potential solution, offering the prospect of home-based care and autonomous medication administration through an epidural continuous block with intermittent drug injection. Conclusions: This case highlights home-based interventional pain management for cancer, addressing challenges at the patient, health professional, and system level. Overcoming these challenges requires a multidisciplinary approach and robust institutional policies. The insights gained offer valuable lessons for hospitals aiming to enhance the competence of pain and palliative care teams and establish comprehensive support systems in hospital and home settings.

Abstract

Abstract—COVID-19 has induced a global health problem with the impact to different aspects of human life. If numbers of patients are increasing beyond the healthcare capacity, the constraint of resources will stimulate ethical dilemma. In hospitals, the availability of drug, hospital beds, trained health professionals, and personal protective equipment are not enough to response patients’ needs. To face this condition, health professionals and hospitals need ethical guidance to allocate resources. This article aims to discuss the guides that are available in the international literature and to recommend Advanced Care Planning as an additional measure to manage the scarcity or resources. The communal culture or Indonesia may contribute to the ethical challenges through the ‘in-group’ and ‘out-group’ phenomenon. Keywords: COVID-19, resource allocation, ethical dilemma Abstrak—Pandemi COVID-19 telah menyebabkan masalah kesehatan global yang berdampak ke berbagai aspek kehidupan manusia. Jika angka penderita melebihi kapasitas sistem layanan kesehatan, terjadi keterbatasan sumber daya yang akan menimbulkan dilema etik. Di rumah sakit, ketersediaan obat, tempat tidur, tenaga kesehatan terlatih, alat pelindung diri tidak cukup untuk merespons kebutuhan pasien. Untuk menghadapi kondisi ini, tenaga kesehatan dan rumah sakit membutuhkan panduan etika untuk mengalokasikan sumber daya. Artikel ini bertujuan untuk mendiskusikan panduan-panduan yang ada di internasional serta mengusulkan Advanced Care Planning sebagai pendekatan tambahan dalam mengelola keterbatasan sumber daya. Budaya komunal di Indonesia dapat menambah tantangan etika di Indonesia melalui fenomena ‘dalam’ dan ‘luar’ kelompok. Kata kunci: COVID-19, alokasi sumber daya, dilema etik
