Abstract

Introduction: Data from the Kidney Foundation Diatrans Indonesia stated that the number of patients with renal failure in Indonesia is estimated to increase 4,400 patients each year. One of the adverse effects of chronic kidney disease is the disorder of blood filtration. Interventions applicable to patients with impaired filtration include hemodialysis therapy. Complications that often occur in patients undergoing hemodialysis are hemodynamic disorders, especially blood pressure to drop or intradialytic hypotension. Objective: This study aims to review some literature on the effect of ultrafiltration rate on blood pressure of patients with chronic kidney disease who undergo hemodialysis. Method: Journal article search is done electronically using multiple databases:  PubMed database, Google Scholar, Science Direct and Mendeley from January 2007 to November 2017. Keywords used were "Ultrafiltration / Ultrafiltration rate", "Blood Pressure", "intradialytic intradialytic / intradialytic hypertension", "Hemodialysis", "Chronic Kidney Disease / CKD" to obtain three articles for review. Result: Ultrafiltration rate can be expressed as having an effect on blood pressure in a hemodialysis patient. Discussion: When hemodialysis, ultrafiltration is done to attract excess fluid in the blood. The amount of fluid secreted during hemodialysis can affect the drop in blood pressure due to a decrease in the volume of blood being pumped. This is related to the reduced volume of strokes produced by the heart. Conclusions: Ultrafiltration rate affects the blood pressure of patients undergoing hemodialysis.

Abstract

Introduction: Asthma is one of the most frequently occurring chronic diseases and can be experienced by people of all ages. In asthma patients, problems with less effective airway clearance can arise due to airway spasm. The main management of asthma is maintaining asthma control using asthma management interventions. This case report aims to determine the effect of the butekyo breathing technique on peak expiratory flow (PEF) and oxygen saturation in asthma patients. Purpose: This systematic review aims to determine the effect of the butekyo breathing technique on peak expiratory flow (PEF) and oxygen saturation in asthma patients. Methods: Implementation is carried out based on Evidence Based Practice (EBP). The EBP found concluded that the butekyo breathing technique is a nursing intervention that can be used as an asthma management intervention. Butekyo breathing technique intervention was carried out twice a day for 14 consecutive days. Evaluation of results obtained from monitoring peak expiratory flow (PEF) values using a peak flow meter and oxygen saturation using oxymeter. Discussion: Based on the results of measuring the peak expiratory flow (PEF) value, the APE value before the Butekyo breathing intervention was carried out was an average of 80% to 137.5%. The highest oxygen saturation value appeared at 98%. Conclusion: The results of the implementation that has been carried out are in accordance with evidence based practice (EBP), namely that the butekyo breathing technique has an effect on increasing APE. Patients who had moderate category asthma experienced an increase in peak expiratory flow of 57.5%.

Abstract

Introduction: Fracture is a loss of continuity of bone, either total or partial, is usually caused by trauma, which is characterized by a history of trauma fracture, pain and swelling in the broken bones, deformity, musculoskeletal dysfunction, breaking the continuity of the bone, and neurovascular disorders. Purpose: The purpose of this study is to identify the effectiveness of music therapy in reducing pain scale on postoperative patients with fracture so it can be added to the understanding of nursing in caring for patient with postoperative fracture pain. Method: This systematic review was carried out according to the PICO model. Keywords in the search for evidence based in this literature review include: “pain scale”, “music therapy”, “postoperative”, “fracture”. Database searches or databases were carried out in December 2022 using Google Scholar, PubMed, Neliti, and Genius. Result: Music therapy applied to the patient with postoperative fracture can reduce the pain scale level. Discussion: One of the treatments from the nursing field that can help reduce pain scale level in patients with postoperative fracture pain is music therapy. Conclusion: Based on the analysis that has been done by the author, it can be concluded that music therapy helps to reduce the pain in postoperative fracture by the activation of brain stem reticulum because a signal to inhibit pain is sent to the spinal cord, and this causes the gateway to close.

Abstract

Introduction: Hypertension is an abnormal increase in blood pressure that can cause persistent symptoms in target organs. Uncontrolled hypertension can lead to complications such as heart disease, kidney failure, and stroke. Nonpharmacological therapies that can be done to reduce blood pressure are slow deep breathing and music therapy. Purpose: To determine the effect of a combination of slow deep breathing therapy and music therapy on lowering blood pressure in the elderly with hypertension. Methods: The research design used a case study method by providing slow deep breathing and music therapy interventions for 7 days to 2 case study subjects who experienced hypertension. Data collection using observation sheets and standard operating procedures (SOP) slow deep breathing and music therapy. Blood pressure measurement tools use a digital Sphygmomanometer and data analysis using descriptive analysis. Discussion: The implementation results showed that after the combination of slow deep breathing therapy and music therapy for 7 days there was a decrease in blood pressure in both case study subjects. The decrease in blood pressure in case study subject I on the first day after therapy was 198/103 mmHg and after 7 days of therapy it became 178/101 mmHg. In case study subject II, on the first day of intervention, blood pressure was 163/97 mmHg and after intervention for 7 days it became 152/95 mmHg. Conclusion: Combination therapy of slow deep breathing and music therapy can reduce blood pressure in elderly people suffering from hypertension.

Abstract

Introduction: Chronic rhinosinusitis is a disease with a high incidence rate, this disease also has an impact on reducing the productivity and quality of life of sufferers. Rhinosinusitis can be caused by multifactorial causes, but most are also caused by inflammation due to viral infections or allergies, resulting in mucosal edema which results in obstruction of the sinus ostium and impaired drainage of secretions. Purpose: The purpose of this study was to determine the effect of nasal irrigation or nasal washing using saline solution on the symptoms complained of by patients and on the condition of airway clearance from patients, so that it can become additional insight for nurses in caring for patients with rhinosinusitis. Methods: This systematic review was conducted according to the PICO model. Keywords in the search for evidence based research in this literature review include: “nasal irrigation” “rhinosinusitis” A database search was conducted in November 2022 using the Google Scholar, PubMed, Science Direct database. Results and discussion: Based on a review of several journals regarding the implementation of nasal irrigation with saline solution, it was found that nasal irrigation using saline solution had a better effect than standard therapy without nasal irrigation with 0.9% NaCl. In addition, it provides good symptom improvement compared to anti- allergy treatment in patients, improves quality of life, and prevents disease recurrence. Conclusion: Based on the results of 10 journal analyzes that have been conducted by the authors, it can be concluded that nasal irrigation using saline solution is proven to be effective in cleaning the nose of accumulated secretions, crusts, and irritant components, thus improving mucosal cleaning. In addition, nasal irrigation has advantages over other techniques such as nasal sprays.

Abstract

Background : Hypertension is a condition when systolic blood pressure is ≥140 mmHg or diastolic blood pressure is ≥90 mmHg. One of the signs and symptoms of hypertension is a stiff neck or neck pain. Non-pharmacological management of hypertension can be done with Progressive Muscle Relaxation (ROP). Purposes : To identify the effect of implementing progressive muscle relaxation in reducing blood pressure in hypertension sufferers in Banteran Village. Methodology : The method used in this final scientific work is a case report in the form of applying Evidence Based Practice (EBP) Progressive Muscle Relaxation (ROP) to Mrs. S once for six days and the patient continues to use pharmacological therapy to lower blood pressure. Results: Implementation results show that ROP has an effect on reducing blood pressure in Mrs. S. Blood pressure on the first day before ROP was 165/95 mmHg, after ROP was 144/88 mmHg. On the sixth day, blood pressure before ROP was 138/79 mmHg, then after ROP it became 125/77 mmHg. Conclusion : The ROP given to Mrs. S has a positive influence where the patient's blood pressure decreases every day.

Abstract

Introduction: Diabetes mellitus is also known as a "lifelong disease," but it can be managed. Indonesia ranks fifth in the world for the highest number of sufferers, with 19.5 million sufferers in 2021, and this number is predicted to increase by 2045. Management is needed to improve the quality of life and prevent complications in diabetes mellitus sufferers. Foot exercises are considered a simple treatment option, but many people are unfamiliar with it. Therefore, health education is needed using media that can increase knowledge, such as booklets. Objective: This study aims to determine the effect of implementing health education in the form of diabetic foot exercises using booklets as media on diabetes mellitus sufferers. Methods: This research method used a case report and the DKQ-24 (Diabetes Knowledge Questionnaire), a questionnaire assessing knowledge about diabetic foot exercises. Descriptive data analysis was then conducted by comparing pretest and posttest scores. Discussion: After health education using a booklet during one meeting and demonstration of diabetic foot exercises during three meetings, there was an increase in respondents' knowledge as evidenced by an increase in posttest results. Conclusion: Implementation of health education on diabetic foot exercises using booklets for Mrs. TM can increase the knowledge of diabetes mellitus sufferers.

Abstract

Background:  The trend of diabetes cases has shifted to younger ages, starting at the age of 15 years old, due to unhealthy lifestyles. Researchers have discovered that health students also tend to have a poor lifestyle. Students are associated with emotional activity and busyness, but no research has examined the relationship between these conditions and diabetes prevention behavior. Purpose: This study examined determinant factors affecting diabetes prevention behavior in college students with diabetes mellitus risk.Methods: This study used a cross-sectional design, utilizing random sampling to gather data from 209 college students with diabetes risk. The instruments used in this study included adaptations of the Diabetes Prevention Behavior questionnaire, the modified Diabetes Management Time Questionnaire (DMTQ), the Perceived Behavioral Control, and the Trait Emotional Intelligence Questionnaire. Data analysis encompassed One-way ANOVA, Mann-Whitney, Kruskal-Wallis, Spearman’s rank, and multiple linear regression.Results: The study found a significant relationship between diabetes management time (p=0.001) and emotional intelligence (p=0.000) with diabetes prevention behavior. Emotional intelligence emerged as the dominant influencing factor (B=0.332). Conversely, no significant relationships were observed between perceived behavior control (p=0.223), class (p=0.734), sex (p=0.231), study major (p=0.263), age (p=0.064), and diabetes mellitus risk (p=0.664) with diabetes prevention behavior.Conclusion: This study revealed a significant relationship between diabetes management time, emotional intelligence, and diabetes prevention behavior. Emotional intelligence emerged as the key factor affecting diabetes prevention behavior, highlighting the need for focused interventions to enhance emotional intelligence and improve diabetes management among college students.

Abstract

Introduction: Diabetes Mellitus (DM) is a chronic disease that is sweeping the world. More than 300 million people in the world suffer from this disease and the number is increasing rapidly. Management and treatment of diabetes involves both pharmacological and non-pharmacological methods. One of the non pharmacological methods is acupressure. Purpose: to find out the benefits of acupressure therapy in health practice and it is hoped that this study can be an illustration of alternative therapies to overcome health problems such as type II diabetes mellitus. Methods: This systematic review was carried out in accordance with the PICO model. The database or database search in this literature review was carried out in April 2022. The database used was Google. A total of 5 articles were used as research samples that met the inclusion and exclusion criteria. Results: 4 out of 5 articles analyzed stated that acupressure was considered to help lower blood glucose levels. Meanwhile, in one article, it was stated that there was no significant difference between the mean glucose levels of the three groups before and after the intervention. Discussion: Self-acupressure at 4 points LIV 3, ST36 SP6 and KD 3 under controlled conditions can increase insulin levels. Acupressure therapy was carried out routinely at ST 36 points and Suzana's feet 3 times on the 6th day, 12th day and 18th day. Emphasis on points ST-36 and SP-6 can restore the balance of Yin and Yang and improve insulin secretory function in patients with non-insulin dependent diabetes mellitus. Conclusion: Acupressure therapy can reduce blood glucose levels in type II DM patients. Acupressure therapy must be carried out according to the correct and regular procedure to avoid failure in acupressure therapy.
