Abstract

This research aims to analyze the implementation of continuous professional development in hospitals. The method used is a case report including assessment, problem identification, problem analysis using fishbone, preparation of a Plan of Action (PoA), implementation, and evaluation. The research results show that the main problem is that continuous professional development could have been more optimal. The problem was solved using Kurt Lewin's planned change approach, with the implementation preparing a draft guide for implementing sustainable professional development. Conclusion: The situation in hospital management is that the performance of continuous professional development could be more optimal. The strategy to optimize the implementation of ongoing professional development is to create a guide that becomes a reference for nurses following the continuous professional development program. Abstract

This research aims to describe the implementation of planned changes to improve the implementation of stunting risk discovery, through the preparation of guidelines for stunting and wasting risk nursing care. The research method used is a case report which includes problem identification, problem analysis, plan of action (POA), implementation and evaluation activities. Data collection was carried out using interview guides, questionnaires. The results of the research show that the implementation of the planning function in implementing the stunting program is not yet optimal, so guidance is needed so that implementation is more optimal. In conclusion, the preparation of stunting risk nursing care guidelines and clinical pathways is a solution to the problem. PoA implementation was carried out using Kurt Lewin's planned change approach. Keywords: Nursing care, Clinical pathway, Stunting

Abstract

Background: One of the major health challenges in the 21st century is managing chronic diseases, which place a burden on every country. The frequency of repeat care for patients with chronic conditions is rising, resulting in increased medical and treatment costs for the state. The World Health Organization suggests discharge planning through continuity of care (COC), which involves consistent communication and coordination to distribute the workload evenly among healthcare teams. Purpose: To identify the benefits of a continuous nursing system for patients. Method: Applying a literature review following the guidelines of the Preferred Reporting Items for Literature Review and Meta-Analyses (PRISMA). Articles were gathered from online databases including Proquest, SAGE Journals, Science Direct, and Wiley Online. The keywords used were "affect," "continuity of care," and "patient." Articles were selected based on the following inclusion criteria: focus on the benefits of a continuous nursing system for patients, publication years between 2010 and 2021, and use of cross-sectional, cohort, mixed research, and review methods. Results: The benefits of a continuous nursing system include lower mortality rates, reduced risk of complications, better quality of care, reduced treatment costs, improved interpersonal relationships, higher patient satisfaction, and enhanced bodily functions. Conclusion: A continuous nursing system offers significant benefits to patients and can be adopted across all healthcare services and patient groups.
