Abstract

Sistem perawatan kesehatan yang dibebankan tenaga kesehatan di setiap fasilitas kesehatan selama pandemi COVID-19, menyebabkan tenaga kesehatan mengalami kelelahan, stres fisik dan mental yang akan berpotensi menurunnya kepatuhan terhadap tindakan pencegahan dan pengendalian penyakit infeksi yang direkomendasikan. Penelitian ini bertujuan untuk mengidentifikasi hubungan kepatuhan penggunaan APD dengan shift kerja perawat selama masa pandemi COVID-19 di salah satu Rumah Sakit Umum di Kabupaten Malang. Penelitian ini dilakukan bulan November sampai Desember 2022 dengan menggunakan pendekatan cross sectional. Responden yang terlibat dalam penelitian ini sebanyak 90 perawat dengan menggunakan metode purposive sampling. Penelitian ini menggunakan lembar observasi untuk mengetahui kepatuhan penggunaan APD setiap responden. Uji yang digunakan adalah uji Chi Square untuk melihat hubungan antara variabel independent dan dependent. Rata-rata usia responden adalah 29,13 tahun dan mayoritas responden adalah perempuan. Sebesar 57,6% responden patuh dalam penggunaan Alat Pelindung Diri (APD) selama mereka bekerja. P-value didapatkan 0,036 maka didapatkan hasil terdapat hubungan yang signifikan antara kepatuhan penggunaan alat pelindung diri dengan shift kerja. Shift kerja mempengaruhi kepatuhan penggunaan APD pada perawat. Tingkat kepatuhan shift pagi lebih tinggi daripada shift sore, dan shift sore lebih tinggi dari shift malam.

Abstract

Most COVID-19-positive patients with comorbidities, especially HT (hypertension), are still being debated. Self-management is very useful for improving the quality of life and changing behavior. This study aimed to determine the self-management experience of COVID-19-positive patients with comorbid hypertension. The method used in this study is qualitative research with a case study approach strategy (case study research), using the nursing process approach, obtained using purposive sampling, interviews, physical assessments, and environmental observation. The results of this study obtained self-management carried out by patients: stress management and lifestyle. Even though self-management has been carried out, they are still exposed to COVID due to age and obesity. Discipline toward self-management prevents the transmission of the virus.

Abstract

Obstructive sleep apnea (OSA) is a health problem that can cause various negative impacts on health, but society considers OSA to be normal. This research aims to determine how the patient's perception and efforts reduce OSA risk. A qualitative study with a phenomenological approach was conducted. Inclusion criteria was experiencing OSA symptoms such as snoring, sleepiness during the day, and often waking up at night. Seven participants met these criteria were taken by purposive and snowball sampling technique. Data were collected through interviews, Epworth Sleepiness Scale (ESS), and STOP-BANG questionnaire (SBQ). The method and time triangulation were used in this study. Data were analyzed using thematic analysis from the Colaizzi approach. Based on the SBQ, all participants have a high risk of OSA, but they perceived OSA as a normal complaint. Three themes were found in this study: participants perceive OSA as a usual complaint, have a perception that OSA does not endanger health, and do not take action to reduce OSA. It is hoped that the results of this study can increase the awareness of nurses to provide education about sleep problems to the community because sleep problems have not received much attention in Indonesia.

Abstract

Introduction: Quality of life (QoL) in the elderly is shaped by physical, psychological, and spiritual factors. Depression commonly affects older adults and is linked to poorer QoL, while spirituality can offer resilience. However, the combined impact of depression and spirituality on elderly QoL is seldom explored.Purpose: This study aims to investigate the relationship between depression, spirituality, and QoL among elderly individuals in nursing homes, examining how these factors influence their physical, social, and mental well-being.Methods: This study used a cross-sectional study design conducted on nursing home residents. The study population was elderly living in nursing homes, with a sample size of 73 people selected by convenience sampling and calculated using G*power software. Quality of life (QoL) was measured using the SF-36 questionnaire, spirituality was measured using the Spiritual Well-Being Scale (SWBS), and depression was measured using the Beck Depression Inventory-II (BDI-II). Data analysis was performed using multiple logistic regression to analyze the relationship between the study variables.Results: Mean scores for physical and mental components of QoL were 50.06 (±6.80) and 50.22 (±8.73), respectively. Spirituality was positively associated with QoL, especially in social (AOR 1.15) and mental health (AOR 1.06) dimensions (p < 0.05). Depression was negatively related to vitality (AOR 1.11) and mental health (AOR 1.09), showing that greater spirituality aligns with higher QoL, while depression correlates with poorer QoL.Conclusion: Spiritual well-being and depression significantly impact elderly QoL, particularly in social, vitality, and mental health aspects.

Depression generally affects older adults and is associated with a poor life, with sociodemographic and psychological factors such as social support and loneliness playing a role in the onset of depression. However, the relationship between these factors and depression in the elderly is still rarely explored.Purpose: This study aims to explore the relationship between psychological and sociodemographic factors on the level of depression in the elderly living in nursing homes, with a focus on social support, level of loneliness, and cognitive impairment.Methods: This study used a descriptive cross-sectional design and involved 72 older adults living in four East Java, Indonesia nursing homes. Data were collected through questionnaires using the Multidimensional Scale of Perceived Social Support (MSPSS), UCLA Loneliness Scale, Mini-Mental State Examination (MMSE), and Beck Depression Inventory-II (BDI-II), which were translated into Bahasa Indonesia. Multiple logistic regression was used to understand the relationship between sociodemographic and psychological factors and the level of depression in the elderly.Results: Marital status (AOR 0.177, p = 0.030) and social support (AOR 0.950, p = 0.040) are significantly associated with depression in the elderly.Conclusion: Marital status and level of social support have the potential to cause depression in the elderly living in nursing homes.

Abstract

Restless Legs Syndrome (RLS) is a common but often undiagnosed neurological disorder, characterized by an urge to move the legs accompanied by uncomfortable sensations, especially during rest at night. This condition is common in the elderly, disrupts sleep, and negatively impacts mood, energy, and quality of life, especially in nursing home residents. This study investigated the relationship between Restless Legs Syndrome (RLS) and quality of life in the elderly living in nursing homes. The research design was cross-sectional, with a consecutive sampling technique. The independent variable in this study was the severity of RLS measured using the IRLSSG questionnaire. Higher scores of IRLSSG and SF-36 indicated severe RLS and better QoL. Data analysis was conducted using the Spearman correlation test to assess the relationship between RLS and quality of life. The sample used in this study was 70 participants, calculated using G power software. The results showed that the average age of the participants was 75.41 years, with the majority of participants being women. Most participants experience mild RLS (67.14%). Statistical tests showed a significant negative association between RLS and QoL, specifically in the bodily pain (BP) domain, with a p-value of 0.04 and r= - 0.2465, which means increasing RLS will increase BP. Participants experienced mild pain, while 24.29% reported moderate pain, and 8.57% experienced severe pain. Statistical tests showed p value of 0.05, which indicates a significant relationship between the severity of RLS and quality of life. RLS has a significant impact on the quality of life in the elderly. Therefore, healthcare providers need to recognize and manage RLS symptoms to improve the overall well-being of this population. For further research, researchers need to consider additional factors such as comorbidities and social support, and implement interventions to reduce RLS symptoms.

Abstract

Kebiasaan tidur yang tidak baik dan gangguan tidur dapat meningkatkan resiko terjadinya berbagai macam penyakit dan kematian. Angka kejadian kengantukan berlebihan di siang hari (excessive daytime sleepiness) yang tinggi dan korelasinya bervariasi pada lansia terutama yang tinggal di lingkungan komunitas. Tujuan penelitian ini adalah untuk mengidentifikasi prevalensi kengantukan berlebih di siang hari pada lansia dan faktor yang mempengaruhinya. Design penelitian adalah cross-sectional study, dan sampel diambil dengan teknik consecutive sampling. Data demografi terkait usia, jenis kelamin, lama tinggal di panti, konsumsi kafein, indeks massa tubuh, dan riwayat penyakit. Untuk mengukur kengantukan berlebih pada lansia, peneliti menggunakan the Epworth Sleepiness Scale (ESS). Penelitian ini melibatkan 70 lansia dari panti wreda di Malang, Jawa Timur. Rata-rata usia responden adalah 75.41±9.50 tahun, dengan mayoritas responden adalah berjenis kelamin perempuan sebanyak 78,57%. Responden memiliki lama tinggal di panti selama rata-rata 3.03±3.37 tahun. Indeks Massa Tubuh (IMT) lansia adalah 21.8±4.2 kg/m2 dan berada pada kategori normal. Sebanyak 54,29% lansia memiliki berbagai macam riwayat penyakit seperti hipertensi, diabetes mellitus, dll. Prevalensi kejadian kengantukan berlebih pada lansia adalah sebesar 5.71% sedangkan mayoritas lansia tidak mengalami kengantukan berlebih di siang hari (66%). Kantuk berlebih pada lansia perlu ditangani dengan berbagai macam strategi penanganan yang konprehensif dan sangat penting untuk meningkatkan kualitas hidup lansia.

Abstract

Hypertension is a major non-communicable disease often referred to as a silent killer due to its asymptomatic nature and potential to cause serious complications. Walking exercise is one of the recommended non-pharmacological interventions proven to help reduce blood pressure. This case study, using a nursing care approach, aims to examine the effect of walking therapy on lowering blood pressure and reducing headache intensity in a 62-year-old female patient living in Pakis Village, Malang Regency. The intervention consisted of a 30-minute daily walking session for seven consecutive days. The results showed a decrease in blood pressure from 160/95 mmHg to 140/85 mmHg, along with a reduction in headache intensity from a score of 6 (moderate pain) to 2 (mild pain). Physiologically, walking improves vascular elasticity, reduces stress hormones, and stimulates the release of endorphins that act as natural analgesics. These findings indicate that walking therapy is an effective, safe, and practical non-pharmacological nursing intervention to support hypertension management and improve the quality of life in older adults.

Abstract

Posyandu merupakan wadah masyarakat yang dikelola dan diselenggarakan dari, oleh, untuk dan bersama masyarakat dalam suatu wilayah. Program UKMB (Upaya Kesehatan Berbasis Masyarakat) dimana kader Posyandu merupakan key person dalam program ini.   Mengurangi terjadinya kecacatan dan penurunan angka kematian merupakan tujuan dari program UKMB. Sehingga diharapkan improve skill dan pengetahuan bagi para kader Posyan sangat dibutuhkan.  Desa Torongrejo – Junrejo memiliki 2 posyandu Lansia dan 4 posyandu Balita sesuai dengan batasan desa.   Hasil pendataan melalui observasi dan wawancara yang dilakukan oleh Tim pada bulan Februari 2021 diketahui bahwa para kader belum perna dilakukan pelatihan PPGD (Pertolongan Pertama gawat darurat) dan penggunaan alat screening yang diberikan oleh Puskesmas. Selama ini kader selama ini saat menolong korban kecelakaan menggunakan teknik semampunya sesuai pengalaman masing-masing. Seperti kita ketahui jika dalam pemberian pertolongan pertama penolong melakukan kesalahan justru korban akan mengalami kecacatan hingga kematian. Hasil musyawarah dengan Mitra solusi yang diberikan Tim antara lain: 1. Kaderisasi pengurus Posyandu, 2.Pelatihan dan pendampingan penggunaan alkes untuk membantu Tim kesehatan dalam melakukan screening, PPGD (penanganan pada korban kecelakaan,  kena air panas, BLS, Choking untuk bayi dan dewasa), 3. Pelatihan pada kader dan anggota keluarga yang mempunya keluarga terkena stroke untuk melakukan ROM pasif dan active, self-management agar tidak terjadi kekambuhan berulang. Metode pendampingan dan pelatihan ini dilakukan di balai desa diwakili oelh 12 orang dari Posyandu Balita dan Lansia. Pelatihan dilakukan 2 hari  pada hari 1 dilakukan pelatihan tentang PPGD, sedangkan hari kedua edukasi pencegahan stunting pendekatan pada gizi dan pelatihan alkes.Â
