Abstract

Osteoarthritis (OA) is the most common rheumatologic disease and one of the leading causes of disability worldwide. Hand OA is the most common OA types after knee and hip OA. Risk factors include age, obesity, and family history. Abnormal mechanical loads, sex hormones, and inflammatory changes play role in pathogenesis of hand OA. Diagnosis is primarily based on clinical examination according to ACR guidelines. Plain radiography may support the diagnosis. ACR 2019 recommends comprehensive individualized treatment which include single or combination of physical, psychosocial, and/or pharmacological intervention. Topical NSAIDs are conditionally recommended. For initial oral medication, oral NSAIDs are strongly recommended and preferable to other medications. Exercise is also strongly recommended. Hand orthoses are highly recommended for patients with first CMCJ OA and conditionally recommended for patients with OA in other joints of the hand.Osteoartritis (OA) adalah penyakit reumatologis paling umum dan salah satu penyebab utama kecacatan di seluruh dunia. Osteoartritis tangan adalah jenis OA yang paling umum setelah OA lutut dan pinggul. Faktor risiko penyakit ini adalah usia, obesitas, dan riwayat keluarga. Beban mekanis abnormal, hormon seks, dan reaksi inflamasi juga berperan pada patogenesis OA tangan. Diagnosis OA tangan dapat ditegakkan klinis sesuai pedoman ACR. Pemeriksaan radiologis dapat membantu diagnosis. Pedoman ACR 2019 merekomendasikan tatalaksana komprehensif tunggal atau gabungan dari intervensi fisik, psikososial, dan/atau farmakologis sesuai kebutuhan pasien. OAINS topikal direkomendasikan untuk OA tangan pada kondisi tertentu. Untuk pengobatan oral awal, OAINS oral direkomendasikan sebagai pilihan utama. Olahraga sangat direkomendasikan termasuk untuk OA tangan. Orthosis tangan sangat dianjurkan untuk pasien CMCJ OA awal dan direkomendasikan pada pasien OA persendian tangan lain dengan kondisi tertentu.

Abstract

Non-cirrhotic portal hypertension (NCPH) merupakan hipertensi portal tanpa sirosis, terjadi pada 10% pasien hipertensi portal. Komplikasi paling sering NCPH adalah varises esofagus yang jug amerupakan penyebab kematian erbanyak.Penyakit ini sulit didiagnosis karena prevalensi rendah dan manifestasi klinis beragam, umumnya varises esofagus, splenomegali dan anemia. Pemerikaan ultrasonografi dan biopsi hepar hasilnya normal. Terapi NCPH adalah beta-blocker untuk mengurangi tekanan portal, terapi endoskopik untuk mengatasi varises esophagus dan operasi.Non-cirrhotic portal hypertension (NCPH) is a portal hypertension that occurs in the absence of cirrhosis, which occurs only in 10% portal hypertensive patients. The most frequent complication of NCPH is esophageal varices that is also the leading cause of death. Diagnosis is difficult because of low prevalence and multiple clinical manifestations, usually as esophageal varices, splenomegaly and anemia, while ultrasonographic and liver biopsy are normal. Treatment is beta-blockers to reduce portal pressure, endoscopic therapy to overcome esophageal varices and surgery.

Abstract

Salah satu penyebab kematian neonatus paling utama adalah asfiksia saat lahir. Tujuan penulisan ini adalah untuk meningkatkan kemampuan dokter, terutama di Indonesia, untuk mendeteksi dan menangani jalan napas sulit pada anak-anak terutama neonatus. Penyebab paling umum jalan napas sulit pada neonatus adalah malformasi kraniofasial. Neonatus dengan jalan napas sulit perlu diberi bantuan ventilasi dengan facemask dan jika masih tidak adekuat, perlu intubasi. Intubasi yang gagal baik dengan laringoskopi direk dan indirek akan berujung kepada trakeostomi.One of the major causes of neonatal death was asphyxia at birth. The purpose of this paper is to improve the ability of physicians, especially in Indonesia, to detect and deal with difficult airway in children especially neonates. The most common cause of difficult airway in neonates is craniofacial malformations. Neonates with difficult airway should be given assisted ventilation with a facemask and if necessary, intubation. Failed intubation both with direct and indirect laryngoscopy will lead to a tracheostomy.

Goji berry merupakan tanaman obat yang mulai dikenal di Indonesia karena dipercaya bermanfaat bagi kesehatan dan karena kandungan antioksidan yang tinggi. Penelitian mengenai tanaman ini masih terbatas.Goji berry is a herbal known in Indonesia for its presumed health benefits and its high antioxidant content. Studies on this herbal medicine are still limited.

Abstract

Introduction: Endometrial cancer is often associated with obesity, type 2 diabetes mellitus (T2DM), insulin resistance, and hyperinsulinemia. Case: A 71-year-old woman with a history of T2DM, hypertension, and obesity (body mass index/BMI 32.08 kg/m²) presented with intermittent abdominal pain and vaginal bleeding. She had undergone a curettage procedure two weeks earlier. Ultrasound examination revealed a hyperechoic mass in the uterine corpus, and histopathological examination following staging laparotomy confirmed type 2 endometrial carcinoma (high-grade serous carcinoma) with omental metastasis. The diagnosis was stage IVB endometrial carcinoma. During hospitalization, the patient received intravenous ceftriaxone, insulin glargine, and oral antihypertensive medications (ramipril and nifedipine). She was scheduled to receive systemic chemotherapy with carboplatin and paclitaxel, the standard regimen for advanced-stage, aggressive non-endometrioid endometrial cancer. Discussion: Diabetes mellitus, particularly when accompanied by chronic hyperglycemia, insulin resistance, and hyperinsulinemia, is associated with an increased risk of various cancers, including endometrial cancer, as well as a poorer prognosis. Factors such as obesity, age, estrogen exposure, and insulin therapy may exacerbate this risk, while biomarkers such as IGF-2 and IGFBP-3 have the potential to be used for early detection and risk assessment of endometrial cancer. Conclusion: This case highlights the strong link between T2DM, obesity, and aggressive endometrial cancer, emphasizing the importance of early detection and comprehensive management in patients with metabolic risk factors.
