Abstract

One of the mainstay drugs to treat infections is antimicrobials, including antibacterials/antibiotics, antifungals, antivirals and antiprotozoa. Antibiotics are drugs used in infections caused by bacteria. Antibiotics that are not used wisely can lead to resistance problems. Therefore, the use of antibiotics must follow a rational strategy of prescribing antibiotics. Irrational prescribing will increase the occurrence of drug side effects, drug interactions, medical costs and result in decreased patient compliance in taking drugs.       The purpose of this study was to determine the pattern of prescribing combinations of intravenous antibiotics in one prescription sheet and their interactions. This research is a type of quantitative research, with descriptive methods and using retrospective data. The population in this study were all prescription sheets of inpatients who had been served at the X  Hospital Pharmacy Installation for the period January 1 to March 31, 2021. The samples were all prescription sheets containing a combination of 2 (two) intravenous antibiotics (injection or infusion), all of which totaling 170 sheets.      The results showed that there were more male patients than female patients, namely 52.3% with the largest age range between 46-55 years (29.41%). Of the 15 combinations of 2 (two) intravenous antibiotics, there were 6 combinations that had moderate pharmacodynamic interactions. Moderate interactions are clinically significant, usually avoided in combination or used only in special circumstances and with close monitoring of health workers. Of the 170 prescription sheets, 135 sheets (79.4%) were prescribed by internal medicine specialists. All intravenous antibiotics prescribed according to the X  Hospital Formulary and the National Formulary.

Abstract

Type 2 Diabetes Mellitus is a non-communicable chronic and progressive disease that can lead to increased blood glucose levels. Patient adherence to medication is essential for achieving therapeutic success. The objective of this study is to determine the relationship between education and medication adherence levels of Type 2 Diabetes Mellitus patients to blood glucose levels in the Emergency Department of the Polri Hospital, East Jakarta. This research method is non-experimental with an observational approach and a cross-sectional design using prospective data from medical records and direct interviews with patients in the inpatient room of Hospital X in Jakarta. The results of the study on 134 patients showed that the characteristics of Type 2 Diabetes Mellitus patients were: 34.3% (the majority) aged between 56 and 65 years, 60.4% female, 63.4% unemployed, 61.9% had elementary school education, and 60.4% had suffered from diabetes for more than 5 years. The most common comorbidity was CHF (Congestive Heart Failure), with 21 patients (15.6%). The level of medication adherence among patients was low, at 64.9%, and more than half (62.7%) had uncontrolled blood glucose levels. Meanwhile, the role of health education in reminding patients was categorized as good, at 91%. Based on the Bivariate Chi-Square analysis, there was a significant relationship between education and medication adherence, with a significance value of 0.000 (<0.005), and also between medication adherence and blood glucose levels (GDS) with a significance value of 0.000 (<0.005). It can be concluded that education affects medication adherence.

Abstract

Clinical pharmacy services are direct services provided by pharmacists to patients in order to improve therapeutic outcomes and minimize the risk of side effects because the aim of using drugs is for patient safety. One of the clinical pharmacy services provided is assessment and prescription services which must be in accordance with clinical requirements, including the absence of drug interactions and patient allergy records. Based on clinical significance, the severity of drug interactions is classified into three, namely Major, Moderate and Minor. This research uses descriptive methods with retrospective data. The population in this study was all outpatient prescription sheets served from 1 October to 31 December 2023. Sampling used simple random sampling and determining the sample size using the Slovin formula so that the sample used was 460 prescription sheets. Identify drug interactions in one prescription sheet using the Drug Interaction Checker application (Drugbank.com). Results The research showed that of the 460 prescriptions examined, 301 prescriptions had the potential for drug interactions (65.4%), and 320 prescriptions (69.6%) completed the drug allergy checklist. The highest level of drug interactions was at moderate severity, namely 102 prescriptions (33.9%), and minor at 88 prescriptions (29.2%), and the lowest at major severity was 15 prescriptions (5%). If we look at the drug items in one prescription sheet, it can be concluded that there are 506 incidents with the highest level of severity at moderate level, namely 254 incidents (50.2%), which should be avoided in combination or only used in special circumstances and with close monitoring of health workers. While there were 219 minor interactions (43.3%); and major 33 events (6.5%), where this drug combination should not have been given.
