Abstract

Introduction. Untreated ureteropelvic junction obstruction would lead to infection and many other complications. The gold standard to treat this condition is pyeloplasty. Nowadays, laparoscopic pyeloplasty has emerged as a potential modality better than open pyeloplasty. Method. Data retrospectively obtained from the medical records of UPJO patients at Persahabatan Hospital, Jakarta. The patient’s age, gender, weight, and BMI noted. Duration of operation, intraoperative blood loss, and complications recorded as the intraoperative status. Length of hospital stay, pain score on the first post-operative day, time to do the daily activity, and return to work recorded as post-operative variables of this study. Results. We included our ten patients who underwent laparoscopic pyeloplasty in our center. The mean of patient’s age is 40.3 ± 17.1 years with 70% male patients. The average body weight and body mass indexes are 62.7 ± 17.8 kg and 24.2 ± 3.9 kg/m2, respectively. Most of the procedures have been done in 225 minutes. Intraoperative blood loss is around 50 mL, with one patient loss his blood around 500 mL. Describing the post-operative outcomes, the average length of stay of the patients is 8.6 ± 1.6 days, with an average visual analog score of 3.5 ± 1.1 on the first post-operative day. The average time needed to recover to daily activity is around 7.3 ± 1.8 days. Conclusion. This study describes our initial experience on laparoscopic pyeloplasty in RSUP Persahabatan Jakarta. The data showed comparable results with other reviews. We need to bring further improvement to our findings along with our experience.

Abstract

Objective: Adreno cortical carcinoma (ACC) is a rare malignancy. Currently, surgical resection offers the best chance of cure with localized tumor. Multimodal therapy including systemic chemotherapy and radiation therapy are often required for locally advanced and metastatic disease aims to decrease these high recurrence rates. Case(s) presentation: A 42-year-old male patient was referred from internist due to mass in left adrenal. Solid mass with calcification on left adrenal gland within size 9 x 11.8 x 11.5 cm was found in MSCT. We performed complete surgical resection (adrenalectomy), and results from pathology anatomy was ACC functional T2N1M0 (stage 3). The patient was planned eight times chemotherapy with etoposide and carboplatin, but he decided to stop the treatment after six times due to no constitutional complaint. We found no residual mass on follow up six months after operation and patient demonstrated a good clinical outcome after one year. Discussion: We perform open adrenalectomy and after surgery mitotane plus etoposide, cisplatin, doxorubicin (EDP) administered as first-line therapy but we only did chemotherapy with etoposide and carboplatin because mitotane was not covered by patient insurance. We chose to not perform radiation therapy due to lesser benefit of adjuvant radiotherapy as evidenced by many studies in term of recurrence-free survival and overall survival. Conclusion: In our case, adreno cortical carcinoma treated with open adrenalectomy combined with 6 times chemotherapy used etoposide and carboplatin demonstrated a good clinical outcome after 1 year.

Abstract

Objective: This study aims to show our first case of female with periurethral condyloma and how we managed it. Case(s) Presentation: A 40-year-old female came with asymptomatic lesion in the genital area for 4 months, without any history of promiscuity. The physical examination showed a mass of 5 x 5 cm located in the periurethral area. The patient underwent mass excisions without any complication, further pathological examination confirmed the diagnosis of CA. Discussion: After operation and discharge there are no sign of recurrence after 6 months follow up. Conclusion: Despite the rare occurrence in periurethral region, clinicians should be aware of CA case in female and for our first case we used mass excision with satisfactory result.

Abstract

Objective: This study aims to present a case of accidental penile fracture without sexual activity. Case(s) Presentation: A 40-year-old guy was sent to our emergency center nine hours prior to admission with oedema and lack of tumescence on the penis. There were no records of sexual activity or masturbation. The patient was treated for a hematoma and a fracture. The patient raised no complaints during his one-week and two-month follow-up visits after the procedure. Discussion: A urological emergency involving penile fracture is uncommon. It is important to avoid unnecessary delays in obtaining medical help. In the current case study, with urgent surgical intervention, corporal tears were repaired. Recent research recommends prompt surgical treatment of penile fractures, as this appears to be associated with fewer complications and better results. Conclusion: A penile fracture that requires immediate surgical treatment can preserve erectile and voiding function. Keyword: Penile fracture, urological emergency, surgical treatment.

Abstract

Urodynamic testing is crucial for evaluating lower urinary tract (LUT) and other urological conditions related to bladder and urethral function. This study aims to profile LUT problems in our center to target specific diagnostic and treatment strategies. This descriptive and retrospective study utilized secondary data from 324 patients who underwent urodynamic evaluation in the Department of Urology, Persahabatan National Hospital, between February 2022 and May 2023. Among patients with suspected benign prostatic hyperplasia (BPH), over half (52.7%) presented with reduced bladder capacity, with 40.2% showing detrusor overactivity (DO) and 37.5% displaying detrusor underactivity (DU). More than half (59.2%) of urinary retention patients suffered from DU with small bladder capacity (43.7%) and low bladder compliance (32.4%). Similarly, in the overactive bladder (OAB) group, 54.5% had reduced bladder capacity, which was also predominant (56.0%) in the urodynamic stress urinary incontinence (USI) group. Among patients suspected of having neurogenic bladder, 73.3% had DU, 48.9% had small blaSadder capacity, and 40% exhibited bladder atony. Furthermore, among the five pediatric patients assessed, three had a small bladder capacity, low compliance, and DU. These findings underscore the significance of urodynamic testing, which can contribute to better evidence-based treatment in managing LUT dysfunction.
