Abstract

Objective: This study aims to evaluate the learning curve of the urologist to perform supine PCNL and the perioperative outcome of patients based on a single surgeon’s experience. Material & Methods: 60 consecutive patients who underwent modified supine PCNL for renal stone were analyzed. A single experienced urological surgeon performed the supine PCNL. Mean operative time, drop in hemoglobin level, stone-free rate, complications, and length of hospital stay were analyzed to evaluate the learning curve of the surgeon. All parameters were compared among all six groups obtained from the 60 cases in chronological order. Besides, the outcomes of supine PCNL were also compared to prone PCNL. Results: Mean operative time from 60 cases of supine PCNL was 100 ± 27 minutes. The mean operative time was decreased over time, particularly after 20 cases. Significantly different mean operative times (89 ± 14 minutes vs. 126 ± 21 minutes, p < 0.001) in the groups of cases 21-60 compared to the group of 1–20 cases were observed. The total stone-free rate for supine PCNL from all cases was 68%. There was no difference regarding the reduction of hemoglobin level, stone-free rate, hospital stay, and complication rate. No major complication was found among study subjects. Supine PCNL showed similar outcome parameters compared to prone PCNL. Conclusion: The surgeon acquired the surgical competencies to perform supine PCNL after 20 cases. The supine PCNL could remove the kidney stone as effective and safe as prone PCNL.

Abstract

Objective: This study aims to present a case of accidental penile fracture without sexual activity. Case(s) Presentation: A 40-year-old guy was sent to our emergency center nine hours prior to admission with oedema and lack of tumescence on the penis. There were no records of sexual activity or masturbation. The patient was treated for a hematoma and a fracture. The patient raised no complaints during his one-week and two-month follow-up visits after the procedure. Discussion: A urological emergency involving penile fracture is uncommon. It is important to avoid unnecessary delays in obtaining medical help. In the current case study, with urgent surgical intervention, corporal tears were repaired. Recent research recommends prompt surgical treatment of penile fractures, as this appears to be associated with fewer complications and better results. Conclusion: A penile fracture that requires immediate surgical treatment can preserve erectile and voiding function. Keyword: Penile fracture, urological emergency, surgical treatment.

Abstract

Urodynamic testing is crucial for evaluating lower urinary tract (LUT) and other urological conditions related to bladder and urethral function. This study aims to profile LUT problems in our center to target specific diagnostic and treatment strategies. This descriptive and retrospective study utilized secondary data from 324 patients who underwent urodynamic evaluation in the Department of Urology, Persahabatan National Hospital, between February 2022 and May 2023. Among patients with suspected benign prostatic hyperplasia (BPH), over half (52.7%) presented with reduced bladder capacity, with 40.2% showing detrusor overactivity (DO) and 37.5% displaying detrusor underactivity (DU). More than half (59.2%) of urinary retention patients suffered from DU with small bladder capacity (43.7%) and low bladder compliance (32.4%). Similarly, in the overactive bladder (OAB) group, 54.5% had reduced bladder capacity, which was also predominant (56.0%) in the urodynamic stress urinary incontinence (USI) group. Among patients suspected of having neurogenic bladder, 73.3% had DU, 48.9% had small blaSadder capacity, and 40% exhibited bladder atony. Furthermore, among the five pediatric patients assessed, three had a small bladder capacity, low compliance, and DU. These findings underscore the significance of urodynamic testing, which can contribute to better evidence-based treatment in managing LUT dysfunction.

Abstract

The aim of this research is to determine the influence of work discipline and work environment on the performance of PT Inkaprima Jaya South Jakarta employees, both partially and simultaneously. The research method used is a quantitative method with an deskriptif approach. The sampling technique used was saturated sampling technique, namely using a sample of 75 respondents from the entire population. The data analysis techniques used are instrument testing, classical assumption testing, linear regression analysis, correlation coefficient analysis, determination coefficient analysis, hypothesis testing. This is proven by the results of the work discipline hypothesis test, which obtained a tcount value of 4,763 > ttable 1,293 and was strengthened by the ? value < Sig.0.05 or a significance value < 0.05 (0.000 < 0.05). The work environment obtained a tcount value of 0.778 > ttable 1,293 and is strengthened by the ? value < Sig.0.05 or significance value < 0.05 (0.439 > 0.05). The research results of work discipline and work environment simultaneously have a positive and insignificant effect on employee performance at PT Inkaprima Jaya South Jakarta, shown by the multiple linear regression equation Y = 19.557 + 0.427X1 + 0.012X2. This is proven by the results of simultaneous hypothesis testing obtained by the Fcount value (11,200 > Ftable 3.12) and reinforced by the significance value < 0.00 (0.000 < 0.05). The results of the coefficient of determination test for work discipline and work environment variables have an influence of 23.7% on employee performance at PT Inkaprima Jaya South Jakarta. Thus H01 is accepted and H3 is rejected. This means that it is proven that there is a simultaneous insignificant influence between work discipline (X1) work environment (X2) on employee performance (Y)
