Abstract

Latar Belakang: Preeklamsia merupakan masalah kedokteran yang serius dan memiliki kompleksitas yang tinggi. Preeklamsia dapat setiap saat mengalami perburukan, sehingga dibutuhkan suatu metode yang mudah digunakan untuk mendeteksi dan mengidentifikasi adanya perburukan. Salah satunya adalah Modified Early Obstetrics Warning Score (MEOWS) yang merupakan salah satu parameter untuk memudahkan mendeteksi secara dini adanya risiko tinggi pada pasien obstetri. Diharapkan perburukan yang terjadi dapat dicegah atau dikurangi risikonya dengan menempatkan pasien preeklamsia berat menurut level of care disesuaikan dengan kebutuhan pasien.Tujuan: Penelitian ini bertujuan untuk mencari cut off point skor MEOWS pada pasien preeklamsia berat dalam menentukan tempat perawatan.  Metode: Penelitian dengan desain kohort retrospektif. Data diambil dari rekam medis, dengan subyek penelitian yang memenuhi kriteria inklusi dan eksklusi. Data penelitian dikumpulkan, diolah dan dilakukan analisis data univariat, bivariat dan multivariat untuk mengetahui hubungan antar variabel.Hasil dan Pembahasan: Sampel pada penelitian ini berjumlah 187 subyek penelitian. Karakteristik skor MEOWS IGD ≥8 berjumlah 70 subyek dan skor MEOWS IGD <8 berjumlah 117 subyek. Pasien yang dirawat di ICU berjumlah 15 subyek dan yang dirawat selain di ICU berjumlah 174 subyek. Hasil perhitungan mendapatkan nilai cut off point skor MEOWS adalah 7,5 (dibulatkan menjadi skor 8). Terdapat hubungan bermakna antara skor MEOWS ≥8 dengan perawatan di ICU (RR 3,34; CI 95%: 1,19-9,38). Terdapat hubungan yang bermakna juga antara skor MEOWS ≥ 8 dengan kejadian bayi IUFD (RR 9,91; CI 95%: 2,098-40,27).Kesimpulan: Pasien preeklamsia berat dengan skor MEOWS ≥8 memiliki risiko untuk dirawat di ICU 3,34 kali lipat dibandingkan dengan pasien preeklamsia berat dengan skor MEOWS <8. Pasien preeklamsia berat dengan skor MEOWS ≥8 memiliki risiko untuk kejadian bayi IUFD 9,91 kali lipat dibandingkan dengan pasien preeklamsia berat dengan skor MEOWS <8.  Kata Kunci: Modified Early Obstetric Warning Score (MEOWS); preeklamsia berat; intensive care unit

Abstract

Abstract : This study explains the effect of government spending on social and health assistance on HDI, household consumption of HDI and education about HDI in Indonesia. This study combines cross-sectional data in 33 provinces with a time series from 2014-2018, with the Regression Panel method with Effective Effect model selection tests. The results showed that: (1) Government expenditure on social assistance has a positive and not significant effect on the human development index in Indonesia, (2) Government expenditure on health has a positive and significant effect on the human development index in Indonesia (3) House consumption the ladder has a positive and significant effect on the human development index in Indonesia, (4) Education has a positive and significant effect on the human development index in Indonesia.Keyword:Government expenditure on social assistance and health, household          consumption, education and human development index.

Abstract

HIGHLIGHTS PE is still one of the contributors to morbidity and mortality which is quite high for pregnant women in Indonesia. The use of NLR and MEOWS modalities as predictors of preeclampsia incidence shows significant potential and can be applied especially in facilities with limited resources.   ABSTRACT Objective: To describe modalities of detecting the severity of preeclampsia (PE) between neutrophil-to-lymphocyte ratio (NLR) levels and Modified Obstetric Early Warning Systems (MEOWS). Materials and Methods: We used a systematic search in PubMed, ScienceDirect, and Google Scholar for fully-accessed English articles, published between 2015 – 2023 that evaluated modalities between NLR levels and MEOWS for detecting the severity of preeclampsia, which resulted in 208 works of literature. Among the studies, we found 14 literature that met the PICO inclusion criteria and included in this study. Results: PE is still one of the high causes of morbidities and mortality in maternal patients. It is important to predict the possible risk of PE events. NLR is one of the markers that become a prognostic modality to determine the severity of PE. MEOWS is one of the tools to assess the level of care of patients who carry out the treatment. NLR and MEOWS can be modalities for predicting PE severity. Conclusion: NLR and MEOWS can be used even with limited resources. The combined use of NLR and MEOWS can be an alternative and potential modality in detecting PE severity.

Abstract

Objective: Omentum pregnancy is an ectopic pregnancy in the abdominal cavity. The event has high morbidity and mortality. The purpose of this case is to present a rare case of abdominal ectopic pregnancy that occurred in a young woman. Methods: Case report. Results: A 22-year-old woman at 16 weeks gestation presented with abdominal pain and clinical shock. Examination results revealed pregnancy outside the womb, with an estimated fetal weight of 193 grams and positive heart activity. The patient underwent laparotomy surgery, revealing that the pregnancy had occurred in the omentum organ with placental attachment. Conclusion: In this case, emergency management began with the patient's reception in the emergency department, followed by laparotomy exploration. After a meticulous surgery, the pregnancy's location was identified in the omentum, and the evacuation of pregnancy products was performed. The patient received treatment for several days until being discharged home. Early diagnosis and interprofessional management are crucial if similar conditions are suspected in the future to prevent morbidity. Keywords: abdominal cavity, ectopic pregnancy, omental pregnancy
