Abstract

Objective: This study is aimed to report and evaluate the experience and learning curve of extraperitoneal Laparoscopic Radical Prostatectomy (LRP) performed in Cipto Mangunkusumo General Hospital Jakarta. Material & method: We collected all data of patients that had been performed extraperitoneal LRP from June 2013 until February 2016, retrospectively. We divided those data into 3 groups with equal number in each group. We compared the preoperative (age, prostate volume, PSA level and prostate cancer staging), intraoperative (surgery time, total blood loss, conversion to open surgery and other organ injury), and postoperative (postoperative length of stay) variables between those 3 groups using ANOVA and Kruskal-Wallis test. Results: We performed 15 extraperitoneal LRP from June 2013 until February 2016 with patient’s mean age was 64.40 (SD ± 5.30) years old and mean prostate volume was 42.39 (SD ± 29.21). In this study, we found significant differences in surgery time, total blood loss, and postoperative length of stay (LOS) in all group with decrease on each intraoperative and postoperative variables. We also found no conversion to open surgery or other organ injury in all patients. Conclusion: The increase of surgeon’s experience and the number of surgery, and the consistency undergo the procedure of LRP demonstrate a decrease in surgery time, intraoperative blood loss and LOS in LRP patients.

Abstract

Abstract

Introduction: Laparoscopic surgery is increasingly prevalent in urology due to improved surgical expertise. This minimally invasive procedure, popularized in the nineteenth century, offers advantages including reduced pain, less bleeding, shorter hospital stays, and improved cosmetic results compared to open surgery. Laparoscopy has expanded to various applications, including nephrectomies, pyeloplasty, ureteral stone removal, nonpalpable testes, and adrenal surgeries. Today, most urological procedures can be performed using laparoscopy, with oncological outcomes comparable to open surgery. Objective: In this retrospective study, we assess urological laparoscopic surgery results and complications in Padang, Indonesia. Material & Methods: A retrospective evaluation was conducted on patients who underwent laparoscopic surgery in Padang, Indonesia, between September 2017 and July 2023. The assessment encompassed preoperative diagnoses, laparoscopic procedure types, operation duration, hospital stays, blood loss, conversion to open surgery, and complications. Results: Of the 155 patients, 105 (67.7%) were male, with a median age of 37 years (range: 1-78). The common laparoscopic surgeries included simple nephrectomy (29%), laparoscopy undescended testis (UDT) (19.3%), renal cyst unroofing (13.5%), and radical nephrectomy (13.5%). Hospital stays ranged from 1 to 7 days, depending on the laparoscopic procedure type. Approximately 12.9% of patients required conversion to open surgery, and no major complications or fatalities were observed. Conclusion: The success and complication rates of laparoscopic surgeries in our city align with those reported in the literature. We firmly believe that technological advancements, increasing experience, and improved patient tolerance make laparoscopic surgery a secure and minimally invasive alternative to open procedures. Keywords: Laparoscopic surgery, minimally invasive surgery, urological surgery.
