Abstract

Congestive heart failure (CHF) is one of the top 10 diseases in the Inpatient of. St. Elisabeth Hospital Semarang. The main cause of CHF is hypertension. CHF patients receive antihypertensive therapy to control their blood pressure. This study aims to determine the decrease in blood pressure of CHF patients and the suitability of blood pressure of CHF patients with their treatment targets according to the Eighth Joint National Committee (JNC VIII) at the Inpatient Installation of St. Elisabeth Hospital Semarang. This research is an observational descriptive study, using retrospective data. Data was taken from medical records of CHF patients in inpatients who received antihypertensive therapy for the period October - December 2017, 40 years or older, with or without concomitant diseases. Based on the results of tracing of medical records it is known that the number of CHF patients who met the inclusion criteria was 60 people, consisting of 30 male patients and 30 female patients. A total of 35 patients (58.33%) were aged over 60 years. A total of 19 patients had concomitant diabetes mellitus type 2 and / or chronic kidney failure. A total of 51 patients (85%) during hospitalization experienced an average reduction in systolic BP of 21.65 mmHg and as many as 37 patients (61.67%) experienced an average reduction in diastolic BP of 13.38 mmHg. The suitability of the patient's blood pressure when leaving the hospital with the target blood pressure according to JNC VIII is 81.67%. Thus the progression of cardiac deterioration in most CHF patients can be inhibited.

Abstract

The medical problems encountered in geriatric patients cause the prescribing to be susceptible to polypharmacy so that the possibility of creating Drug Related Problems (DRPs) is likely. Beers Criteria is one method to avoid the occurrence of DRPs. This study aims to evaluate prescribing of geriatric patients based on the Beers Criteria 2015 at the internal medicine clinic in the outpatient installation of K.R.M.T Wongsonegoro Hospital Semarang. This research was an descriptive observational study using retrospective data from geriatric patients' medical records at the internal medicine clinic in the outpatient installation of K.R.M.T Wongsonegoro Hospital Semarang in the period October - December 2017. A total of 300 medical records were taken at simple random as samples. Data for each patient were only taken once during the study period. The patient is a national health insurance participant and is ≥60 years old. The data obtained was then identified using Beers Criteria 2015. Based on the prescription of 300 geriatric patients in the internal medicine clinic in Wongsonegoro Hospital Semarang, it is known that 88,33% patients received prescription included in Beers Criteria 2015. The five medicine most prescribed are Na diclofenac, omeprazole, digoxin, ranitidine, and diazepam. Medications included in Beers Criteria are medicine that should not be given to geriatric patients so that caution is needed in the use of these medicine.

Abstract

The high incidence of infectious diseases causes the use of antibiotics to increase. Inappropriate use of antibiotics will lead to resistance. Infectious diseases are included in the top 10 diseases in pediatric patients at St. Elisabeth Hospital.Semarang. This study aims to evaluate the use of antibiotics in pediatric patients in the Theresia ward of St. Elisabeth Hospital Semarang. This research is a descriptive observational study using retrospective data. Data were taken from medical records of pediatric patients in the Theresia ward at St. Elisabeth Hospital Semarang, aged 6 months - 14 years, the use of antibiotic therapy during the period October - December 2019. The data obtained were analyzed by calculating the quantity of antibiotic use using the Anatomical Therapeutic Chemical (ATC) and Defined Daily Dose (DDD)) / 100 patient-days methods. Based on the results of tracing medical record data, it was found that the number of pediatric patients in the Theresia room at St. Elisabeth Semarang for the period October - December 2019 is 385. The number of patients who received a single antibiotic was 162. The total value of DDD / 100 patient-days in the Theresia ward of St. Elisabeth Hospital Semarang for the period October - December 2019 is 18.17 with a total length of stay of 1117 days. The antibiotic with the highest DDD / 100 patient-days was ceftriaxone, namely 15.10. While the antibiotics that enter the DU 90% segment are ceftriaxone and cefotaxime.

Abstract

Diabetes Mellitus (DM) Type II is a group of metabolic diseases with hyperglycemia characteristics. The use of oral hypoglycemic drugs (OHO) lasts for a relatively long time. Most of the OHO is excreted through the kidneys. This study aims to determine the Glomerulus Filtration Rate (GFR) of type 2 DM patients who use OHO at the "X" Hospital, District of Demak. This study is an observational descriptive study using retrospective data obtained from medical record data (RM) of outpatient JKN participants with type 2 DM for the period October-December 2020. Inclusion criteria were patients who received OHO for at least one year with or without comorbidities, aged 36-65 years, and had a serum creatinine test. Patients receiving combination therapy with insulin were excluded. LFG is calculated using the Cockroft-Gault formula. From 82 patients, 52 patients (62.2%) were female, 74 patients (90.2%) were more than 45 years old. Patients who consumed OHO for more than 2 years were 47 (57.3%). Patients without comorbidities were 62 (75.7%) people. Patients who experienced a decrease in GFR in the moderate category (stage 3) as many as 24 people (29.3%), a decrease in GFR in the severe category (stage 4) as many as 5 people (6.1%) and 1 person (1.2%) experienced End Stage Renal Disease.

Abstract

Traditional medicine is an alternative treatment for diabetes mellitus. Flavonoids are compounds that have the activity of lowering blood glucose levels. Snake fruit (Salacca zalacca) contains flavonoids. This study aims to determine the antidiabetic potential of snake fruit ethanolic extract (SFEE) on male white Swiss mice. This research is an experimental study that consisted of 2 stages, namely testing the antidiabetic power and testing the hypoglycemia side effects of SFEE. Each stage was tested on 5 treatment groups. The various doses used were 91 mg, 114 mg, and 137 mg of 20g mice. In stage 1, fasting blood sugar checks are carried out on days 0, 3 and 17. Meanwhile, in stage 2, fasting blood sugar checks are carried out on days 1, 7 and 14. The positive control in the stage 1 test was metformin, while the stage 2 test used glibenclamide. The results showed that varying doses of SFEE  91 mg, 114 mg, and 137 mg of 20g mice had antidiabetic power of 62.87%, 68.07%, and 70.98%. The greater the dose of SFEE, the greater its antidiabetic power. However, the larger the dose of SFEE and the longer it is administered, the hypoglycemic effects will occur

Abstract

Hypertension is often asymptomatic and if left untreated can cause complications. Hypertension is an increase in systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg. The blood pressure target for hypertensive patients with type II diabetes melitus (HT-DMTII) is <130/80 mmHg. The risk of complications increases if blood pressure does not reach the therapeutic target. Ngemplak Simongan Health Center, Semarang City has quite a lot of HT-DMTII patients, who undergo routine check-ups every month. This study aims to determine the achievement of blood pressure targets for HT-DMTII patients at Ngemplak Simongan Health Center, Semarang City. This study is an observational descriptive study using purposive sampling technique. The data used is retrospective data obtained from medical record data of HT-DMTII patients for the period August - October 2023. The inclusion criteria used were patients aged ≥36 years, receiving the same treatment for 2 consecutive months and having blood pressure data for 2 consecutive months. The data taken included medical record number, age, gender, blood pressure data for 2 consecutive months in the period August - October 2023, antihypertensives given and duration of use. From 62 patients, it was known that the majority were female patients 72.58%, aged >60 years as many as 58.06% and those who used antihypertensives for more than 5 years as many as 61.29%. A total of 59 people (95.16%) received single antihypertensive therapy. The most widely used antihypertensive was amlodipin (90.32%). There were 20 patients (32.26%) whose blood pressure reached the target.

Abstract

The paradigm changing in health care form “quality” to “quality and safety” aims to improve the quality of life of patient. One of the efforts is to minimize the occurrence of incidents. The largest number of the patient safety incidence is Nearly Injury Incidence, which is 18.53%. The increased risk of NPC can be caused by their presence of polypharmacy. Polypharmacy is often experienced in patients at the Internal Medicine Polyclinic at Roemani Muhammadiyah Semarang Hospital. Therefore, it is necessary to conduct research to determine the number of KNCs in the prescription service of poly internal medicine patients in the Outpatient Pharmacy Installation of Roemani Muhammadiyah Hospital Semarang. The study was a descriptive observational study with prospective data from prescription services for patients with internal medicine at the Outpatient Pharmacy Installation of Roemani Muhammadiyah Hospital Semarang in March 2020. Evaluations were carried out at each stage of prescription service. Stages consist of prescribing, taking drugs, and preparing medicines etiquette, checking. Data were analyzed and grouped according to the type of KNC at each stage of prescription service for patients with internal medicine, the number and percentage of KNC on each prescription sheet. The results showed that there were 25 KNCs on 375 sheets of prescribed poly patients in internal medicine at Roemani Muhammadiyah Hospital Semarang. The percentage of KNC is the drug entry stage of 9 KNC (2.4%), the taking stage of 15 KNC (4%), the preparation stage of 1 KNC (0.27%).

Abstract

Salak seeds (Salacca zalacca) contain several secondary metabolites like as 0.059% flavonoids, 0.082% tannins and 0.063% alkaloids. Salak is a potential plant as a medicinal ingredient, so it is necessary to conduct a drug safety study. Subchronic toxicity test needs to be done in traditional drug therapy because it is given continuously. This study objective was to examine the subchronic toxicity of salak seed extract on liver function in male Swiss strain mice. This type of research is an experimental study with a completely randomized design with a one-way pattern with one treatment factor. There are four treatment groups with a total of 7 mice replications. Variations in the dose of salak seed extract administration were 91 mg/20 gBW of mice, 114 mg/20 gBW of mice, and 137 mg/20 gBW of mice. The toxicity test were SGPT and SGOT values on days 0 and 14. The data tested with one-way ANOVA followed by a t-test. The Salak Seed Extract (EBS) is a viscous extract with a dark brown colour, a distinctive smell of salak, and a chelate taste. The amount of the extraction result was 12.26%. Positive EBS contains flavonoids with a reddish-orange colour. The treatment of Salak Seed Extract ( 91 mg/20 g BW and 137 mg/20 g BW) was not increased significantly the SGPT and SGOT levels for 14 days (p-value 0.896 and 0.072). Although the second dose (114 mg/20 g BW) was increased the SGPT and SGOT levels (p-value 0.002), this enhancement was less than twice

Abstract

Dengue hemorrhagic fever (DHF) threatens people in the rainy season. DHF is one of the major public health problems in Indonesia. These problems encourage an effort to eradicate Ae. aegypti larvae by using natural larvicide. One of the plants that can be used as natural larvicides is avocado seeds (Persea americana Seed) which contain saponin. This study aims to determine the larvacidal activity of avocado seed extract sandgranules with LC50 of 8.87 mg.mL-1 against Ae. aegypti larvae. This study is an experimental research with a complete random design in a unidirectional pattern. The independent variable in this study is the variation concentration of Explotab as a disintegrant agent ie 2, 3, and 4%. Sand granules of avocado seed extract are made by wet granulation method. Results of the larvacidal activity test of avocado seed extract sand granules were analyzed by calculating the percentage of larvae mortality and statistical analysis using SPSS. The results of this study is the larvacidal activity of pure avocado seed extract with LC50 8.87 mg. L-1 is greater than the avocado seed extract sand granules. The addition of Explotab did not affect the total larvae mortality, but there was a significant difference in larvae mortality at 5 hours of sand granules exposure with explotab concentration of 3% and 4% (a significance value <0.05).
