Abstract

Hypertension is a chronic condition that requires long-term management due to its strong association with an increased risk of serious complications when blood pressure is not adequately controlled. One of the key factors influencing the success of hypertension management is patient adherence to antihypertensive medication. However, suboptimal adherence remains a common problem and may lead to poor therapeutic outcomes. This literature review aimed to examine the relationship between adherence to antihypertensive medication and blood pressure control among patients with hypertension. A systematic search of the literature was conducted using the PubMed and Google Scholar databases with relevant keywords combined through Boolean operators. The selected articles were screened based on predefined inclusion and exclusion criteria, including original research articles published within the last ten years, accessible in full text, and addressing the association between medication adherence and blood pressure in hypertensive patients. The findings of the reviewed studies indicate that most articles reported a significant relationship between medication adherence and blood pressure levels. Patients with higher adherence to antihypertensive therapy tended to achieve better blood pressure control compared to those with lower adherence. Although variations in the strength of the reported associations were observed across studies, the overall direction of the relationship was consistent. These results suggest that adherence to antihypertensive medication plays an essential role in achieving optimal blood pressure control. Therefore, this review highlights the importance of continuous efforts to improve patient adherence as part of effective hypertension management strategies aimed at reducing the risk of long-term complications.

Abstract

Hypertension is a global problem, and its prevalence continues to rise to alarming levels. This condition is often referred to as the “silent killer” because patients do not experience symptoms or complaints, so they are unaware that they have hypertension, and many people only find out after complications have already developed. Hypertension is the leading cause of death worldwide each year, particularly among the elderly. This is why the WHO has set one of its global targets for non-communicable diseases as reducing the prevalence of hypertension by 33% between 2010 and 2030 (WHO, 2023). Objective: This study aims to identify the factors influencing the risk of hypertension in the elderly. Methods: A systematic literature review was conducted. Studies were identified through searches on Google Scholar and PubMed for publications from 2019 to 2023 using the keywords “Factors,” “Hypertension,” and “Elderly.” A total of 16,100 results were found on Google Scholar and 959 on PubMed, with 10 journals meeting the eligibility criteria. Results: Based on the journal review, the incidence of hypertension can be influenced by several factors, such as age, genetics, gender, excessive salt and fat intake, obesity, stress, physical activity, smoking, and alcohol and coffee consumption. Conclusion: An individual is more likely to develop hypertension if there is a family history of the condition. Additionally, individuals over the age of 65 with conditions such as obesity and kidney disease are at higher risk of developing hypertension. Once diagnosed with hypertension, an individual must adopt a healthy lifestyle as part of the management of blood pressure control. We can control hypertension risk factors through a healthy diet and lifestyle, avoiding

Abstract

Service quality is one of the key factors that determine patient satisfaction in healthcare services. This study aims to analyze the relationship between service quality and patient satisfaction at a Community Health Center (Puskesmas) using the five SERVQUAL dimensions: tangible, reliability, responsiveness, assurance, and empathy. This research employed a quantitative design with a cross-sectional approach. A total of 54 respondents were selected using a purposive sampling technique. Data were analyzed using the Chi- Square test. The results showed that service quality had a significant relationship with patient satisfaction across all SERVQUAL dimensions, namely tangible (p = 0.006), reliability (p = 0.015), responsiveness (p = 0.015), assurance (p = 0.006), and empathy (p = 0.004). The majority of respondents assessed the overall service quality as good (61.1%), yet dissatisfaction remained in several aspects, particularly in physical facilities, timeliness, responsiveness, and healthcare workers’ personal attention. This study concludes that service quality has a significant association with patient satisfaction, although gaps were found between patients’ perceptions of service quality and their satisfaction in some dimensions. It is recommended that the health center improve facilities and infrastructure, strengthen staff responsiveness and empathy, and optimize communication skills and professionalism among healthcare workers to enhance overall patient satisfaction.

Abstract

One indicator for assessing how community health centers are utilized as healthcare facilities is the number of visits by the community to those centers. A low number of visits indicates that the community is underutilizing healthcare services. Over the past three years, from 2018 to 2020, the utilization of the Silalahi Community Health Center has seen a decline in the number of visits, from 45.8% in 2018, to 38.5% in 2019, and to 33.2% in 2020. This is attributed to the community’s misperception of illness. The objective of this study is to identify the factors associated with the utilization of health services at the Silalahi Community Health Center. This is a descriptive study using a cross-sectional design with a quantitative approach. The sample consisted of 95 respondents; cluster random sampling was used, and a questionnaire served as the data collection instrument. Data were analyzed using the Chi-Square test. The results indicate a significant association between Accessibility (p=0.001), Health Insurance Coverage (p=0.000), and Perception of Illness (p=0.000) with the utilization of health services. Age (p=0.083), Education (p=0.122), Occupation (p=0.093), and Availability of Health Personnel (p=0.368) showed no association with the utilization of health services. Recommendations are provided to the Health Office and Silalahi Community Health Center to further enhance health promotion efforts to improve public knowledge regarding the functions of the Community Health Center and perceptions of illness.

Abstract

The World Health Organization American Heart Association in 2015 stated that currently hypertension sufferers in the world have reached around 970 million sufferers, around 330 million are in developed countries and 640 in developing countries. The purpose of this study is to determine the effect of PROLANIS exercise on reducing blood pressure in hypertension sufferers at the Singosari Pematangsiantar Health Center. This study uses a quantitative method. The design of this study uses the Quasi Experiment method with the research design "One group pretest posttest design". The sample in this study was the elderly as many as 47 respondents. Data analysis was carried out using univariate analysis and bivariate analysis using Wilcoxon. Results: The results of the study were analyzed using a t-dependent test showing that there was an effect of PROLANIS exercise on reducing blood pressure in hypertension sufferers with a P-value of 3,000. This study is expected to maximize the implementation of the PROLANIS program so that the objectives of this activity can be achieved optimally. Conclusion: Implementation of elderly gymnastics can reduce blood pressure in hypertension patients. It can be used as a benchmark for compiling nursing interventions on the influence of PROLANIS gymnastics on reducing blood pressure in hypertension patients at Permatangsiantar Health Center in improving the quality of health services.
