Abstract

Background: Diabetic leg ulcer is a chronic injury caused by peripheral neuropathy and repetitive pressure that results in tissue damage to part or all of the leg area under high pressure. To overcome this, treatment is needed with the application of honey as part of the Moist Wound Healing method. Moist Wound Healing is a wound care method with moist principles that can support cells in the wound healing process. Objective: to see the effectiveness of honey in the Application of Wound Care Procedures Using the Moist Wound Healing Method in Diabetic Leg Ulcer Cases. Method: case study design with descriptive design. Respondents consisted of 2 people who had met the criteria of the researcher. Results: there was a decrease in the REEDA score (Redness, Edema, Ecchymosis, Discharge, Approximation) of respondent 1 from the score (5) to (2), respondent 2 from the score (5) to (2), with an average decrease of 66%. With the state of the ulcer, there is no odor, or exudate, as well as the presence of tissue granulation. Conclusion: the moist wound healing method with honey is effectively used in the treatment of diabetic ulcers. Keywords: Diabetic Leg Ulcer, Wound Care, REEDA, Moist Wound Healing

Abstract

Increasing prevalence of type II diabetes mellitus (DM) leads to various complications, including diabetic foot ulcers, which can risk amputation. Diabetic foot exercise nursing intervention is considered effective as a preventive measure. This study aims to describe the implementation of this intervention in the Masohi Community Health Center. Using a case study design, the subjects included two patients with type II DM for 5-10 years. Data collection involved interviews, vital sign measurements, and diabetic foot ulcer risk assessment using Inlow's 60-Second Diabetic Foot Screening Tool before and after the intervention. The results showed that the first subject experienced a reduction in diabetic foot ulcer risk scores from 8 to 6 on the left foot and from 5 to 3 on the right foot, alongside a decrease in random blood glucose levels (GDS) from 387 mg/dL to 255 mg/dL. For the second subject, the ulcer risk score decreased from 5 to 3 on the left foot and from 8 to 6 on the right foot, with GDS dropping from 339 mg/dL to 250 mg/dL. These findings indicate that diabetic foot exercise effectively reduces ulcer risk and aids in blood glucose control. This study is expected to provide a reference for community health centers and educational institutions in implementing diabetic foot exercise nursing interventions for the prevention of diabetic foot ulcers in type II DM patients.

Sleep quality was measured using Pittsburg sleep quality index form. Datawere analyzed using independent t-test and multivariate of variance with signifinat level 0.05. This study found that Benson’s relaxation therapy was able toimprove significantly the elderly sleep quality (p-value = 0.046), and no significant difference between sex, age and disease suffered in elderly sleep quality(p-value > 0.05). The Benson’s relaxation therapy was able to improve the elderly sleep quality especially in Inakaka Social Institution.

Abstract

Introduction: Nurses, in carrying out their daily duties, deal directly with medical personnel and the patients they handle. There is a demand to provide health services responsibly by applying treatment according to procedures. Collaborative relationships between doctors and nurses are often a complex problem. Nurses work on delegation of authority (delegation, mandate, and other authorities). This act of delegation of authority affects nurse job satisfaction. The purpose of this study was to determine the relationship between the delegation of authority for medical action from doctors to nurses with the job satisfaction of nurses. Methods: A quantitative descriptive design with a cross-sectional approach was applied to this study. The sample size was calculated through appendicitis pre-operative cases and 60 nurses were involved. Data was collected by self-report study and analyzed using the chi-square test. Results: There is a relationship between the delegation of authority and nurse job satisfaction, delegation (0.000), mandate (0.015), and other authorities (0.000). The process of delegation is more on carrying out doctor's orders or instructions written on the patient's status or medical record. Conclusions: There is an appropriate delegation of authority relationship. This is proven by the process of delegating authority; Hospitals did not apply a standard report on delegation. Nurses' job satisfaction was related to service fees that were also shared with doctors. Re-socialize the delegation process, hospitals need to provide delegation standard reports. This strategy might legally support nurses for their independent, collaborative interventions, and nurse's authority as well.
