Abstract

The denture adhesive increases retention on the denture base and affects oral microorganisms. Adding antifungals to denture adhesives can inhibit the Candida albicans biofilms formation and prevent denture stomatitis. The combination of denture adhesives and herbal medicines is an alternative to antifungals, which have few side effects because it is a plant. Moreover, one of them is the endophytic Aspergillus sp. extract containing chemical compounds that can inhibit the Candida albicans biofilms formation. This study aims to analyze the effectiveness of the endophytic Aspergillus sp. extract in denture adhesive materials for Candida albicans biofilm formation on acrylic resin surfaces. The research method is to extract the Aspergillus sp. extract antibiofilm test. Denture adhesive formulation was adjusted to the standard, and added Aspergillus sp. with concentrations of 3.125%, 6.25%, 12%, and 25%. The research sample used hot polymerized acrylic resin. The control group used X denture adhesive and added nystatin, each group suspended by Candida albicans for 24, 48, and 72 hours. Examination of biofilm formation activity on the surface of acrylic resin used SEM. The analysis used Two Way Anova. Aspergillus sp. extract in denture adhesive effectively prevents Candida albicans biofilm formation within 24 hour incubation time. In conclusion, extract of the endophytic Aspergillus sp. in denture adhesive can inhibit the formation of Candida albicans biofilm on the surface roughness of acrylic resin.

Abstract

Prosthetic management for edentulism could be challenging with the Inadequate interocclusal space due to enlargement of the maxillary tuberosity, causes difficulties in artificial tooth placement. Hence, reducing numbers of artificial teeth is indicated which is in line with the shortened dental arches (SDA) concept. The SDA concept could provide good occlusal and mandibular stability, comfort in mastication and appearance. An increase in strain distribution value in the anterior region of the mandible was found in the SDA due to partial changes of the occlusal load distribution pattern. Strain distribution can be analyzed using Finite Element Analysis (FEA). This literature review aims to analyse the strain distribution on SDA complete denture using finite element analysis. The SDA concept was aimed at preserving the anterior and premolar regions. Masticatory ability is correlated with the number of teeth and is impaired if there are less than 20 teeth. Based on FEA, the increase in strain distribution value in the SDA concept is due to changes in occlusal load distribution pattern, masticatory muscle activity and tissue morphology that is susceptible to stress. Nevertheless, the strain value in SDA models were of lower intensity than the yield reported to cause deterioration effects. In maxillary complete denture, the highest strain value was found at the incisal and labial frenal notches. Strain distribution on shortened dental arches using finite element analysis shows a satisfactory masticatory ability and an increase in the value of stress distribution in the anterior region of the mandible.

Abstract

Edentulism is an oral health problem that has an impact on quality of life because it causes a loss of balance in the stomatognathic system with disruption of mastication. The impaired masticatory function can be exacerbated from knife-edge conditions, thus the masticatory load received by the mucous in the form of stress distribution becomes greater. One of the efforts to reduce stress concentration is to modify the occlusion concept and anatomical shape of the artificial teeth. However, measuring the stress distribution on edentulous mucous with various occlusion schemes was difficult with in vivo and in vitro tests due to inability to represent the complex system of stomatognation. In silico test with A) can be a solution because the modeling simulation is obtained from CT-scan or digital design. This paper discusses the stress distribution analysis on knife-edge condition with various occlusion schemes using FEM. Stress distribution that exceeds the mucous pressure-pain threshold during denture function will cause poor mastication performance. This paper discusses the analysis of stress distribution under knife-edge ridge conditions with various occlusion schemes using FEM. The use of Lingualized or Monoplane occlusion schemes can be a solution because they use semi-anatomical and non-anatomical artificial teeth to reduce the load passed on the cusp and also minimize resistance during lateral movement. The advantages of FEM in obtaining accurate modeling and flexibility in testing allow analyses that are difficult to perform in in vivo and in vitro, FEM measure stress distribution and its relationship with pain on knife-edge ridge base on various occlusion schemes in complete dentures.

Abstract

Introduction: The greatest disadvantage of silicone soft denture liner (SDL) materials is the difficulty in keeping them clean due to their incompatibility with conventional denture cleaning solutions. As a result, chemical methods are often recommended as effective alternatives to mechanical cleaning. Natural products have emerged as promising substitutes for synthetic chemical agents. Origanum vulgare, in particular, has shown significant antifungal activity against Candida albicans. This study aims to analyze the effect of incorporating Origanum vulgare gel on auto-polymerized SDL materials, focusing on its impacts on the inhibition zone of Candida albicans over varying usage durations and concentrations. Methods: The antifungal activity was evaluated using the disc diffusion method. The samples were divided into three groups: (1) SDL without antifungal agent addition, (2) SDL with nystatin addition, and (3) SDL with Origanum vulgare gel addition. Data were analyzed using one-way ANOVA and Dunnett’s T3 for usage duration of 7 and 14 days. Results: The addition of Origanum vulgare gel showed significant antifungal activity, with MIC (3.12%) and MFC (6.25%). One way ANOVA analyses the inhibition zone of Candida albicans was significantly affected by the usage duration (7 and 14 days), with p<0.001 respectively. Post-hoc analyses using Dunnett's T3 test for the groups at 7 days and 14 days revealed significant difference in the antifungal effect between each group with MIC and MFC. Conclusion:  Nystatin was found to be less effective than Origanum vulgare gel in inhibiting the growth of C. albicans. The addition of Origanum vulgare Gel at a concentration of 6.25% to auto-polymerized silicone SDL is recommended, as it effectively inhibits the growth of Candida albicans for up to 14 days.

Abstract

Edentulus merupakan kehilangan seluruh gigi permanen pada maksila dan mandibula yang mengganggu mastikasi, namun dapat dihindari dengan menggunakan GTL. Tujuan GTL adalah tercapainya performa mastikasi yang baik. Keberhasilan GTL disebabkan oleh GTL yang retentif dan stabil yang sebagian besar tergantung pada saliva. Laju aliran saliva penting dalam pembentukan adhesi dan kohesi antara basis GTL dan mukosa. Penelitian ini bertujuan mengetahui hubungan laju aliran saliva dengan performa mastikasi berdasarkan jenis kelamin pada pemakai gigi tiruan lengkap. Jenis penelitian ini adalah deskriptif analitik dengan rancangan penelitian cross sectional menggunakan sampel penelitian pemakai GTL dengan oklusi klas I sebanyak 38 orang, dengan kriteria selalu memakai GTL selama siang hari dan ketika makan serta pemakaian minimal 6 bulan pasca pemasangan di RSGM USU. Setiap pasien dilakukan pengumpulan saliva untuk mengukur laju aliran saliva dan pengunyahan Color Changeable Chewing Gum untuk menilai performa mastikasi, kemudian hasil dianalisis dengan uji Chi-Square. Pada hasil penelitian ini laju aliran saliva memiliki hubungan signifikan dengan performa mastikasi pada laki-laki, namun tidak pada perempuan. Laju aliran saliva dapat dijadikan pertimbangan penilaian performa mastikasi pada jenis kelamin laki-laki sehingga dapat digunakan sebagai dasar edukasi hasil evaluasi mastikasi pemakai gigi tiruan lengkap.

Abstract

ABSTRAKPendahuluan: Kehilangan gigi merupakan hilangnya satu atau beberapa gigi pada lengkung rahang yang berdampak pada fungsi estetik, fonetik, dan mastikasi. Mastikasi adalah suatu proses penghancuran makanan menjadi bolus kecil yang mudah untuk ditelan. Fungsi mastikasi dapat dievaluasi dengan performa mastikasi. Performa mastikasi adalah suatu kemampuan individu untuk menghancurkan makanan menjadi bagian-bagian kecil dengan gerakan pengunyahan dan berakhir dengan menelan. Salah satu faktor yang memengaruhi performa mastikasi adalah usia. Sejalan bertambahnya usia, terjadi peningkatan ketegangan dan penurunan elastisitas otot-otot yang ada di seluruh tubuh termasuk otot mastikasi yang akan berpengaruh pada performa mastikasi. Tujuan penelitian adalah untuk menganalisis performa mastikasi pada perempuan pralansia dan lansia pada bilateral free end rahang bawah. Metode: Jenis penelitian ini adalah observasional analitik dengan pendekatan cross-sectional. Rancangan penelitian ini mengambil data performa mastikasi menggunakan metode pencampuran warna dengan mengunyah dua permen karet warna yang berbeda, hasil pencampuran warna ditransformasikan ke dalam software View Gum. Jumlah sampel sebanyak 28 sampel terdiri dari 14 wanita pra lansia berusia 45-59 tahun dan 14 wanita lansia ≥60 tahun dengan teknik purposive sampling dan perhitungan besar sampel menggunakan rumus uji dua proporsi. Hasil penelitian dianalisis dengan uji Mann-Whitney. Hasil: Nilai rerata performa mastikasi pada perempuan pralansia 54,79% sedangkan pada lansia 55,5% serta adanya perbedaan performa mastikasi perempuan pralansia dan lansia pada bilateral free end rahang bawah dengan nilai p=0,03 (p<0,05). Simpulan: Performa mastikasi perempuan pralansia lebih baik dibandingkan lansia pada bilateral free end rahang bawah.Kata kunciperforma mastikasi, bilateral free end, pralansia, lansia, view gum. Differences in the mastication performance of  pre-elderly and elderly women in the bilateral free end of the mandible: cross-sectional studyIntroduction: Tooth loss is the loss of one or more teeth in the arch of the jaw which has an impact on aesthetic, phonetic, and masticatory functions. Mastication is the process of breaking down food into small boluses that are easy to swallow. Masticatory function can be evaluated by masticatory performance. Mastication performance is an individual’s ability to crush food into small pieces with chewing movements and ends with swallowing. One of the factors that affect mastication performance is age. With increasing age, there is an increase in tension and a decrease in the elasticity of the muscles throughout the body, including the masticatory muscles, which will affect the performance of mastication. The purpose of the study was to analyzed masticatory performance in elderly and elderly women and the differences in mastication performance in elderly and elderly women in bilateral free end of the mandible. Methods: This type of research is analytical observational with a cross-sectional approach. This research design took mastication performance data using a color mixing method by chewing two different color gums, the results of color mixing were transformed into View Gum software. The total sample was 28 samples consisting of 14 elderly women aged 45-59 years and 14 elderly women ≥60 years with purposive sampling techniques and calculation of sample size using a two-proportion test formula. The research results were analyzed by the Mann-Whitney test. Results: The average mastication performance in pre-elderly women is 54.79% while in the elderly 55.5% and there are differences in mastication performance in pre-elderly and elderly women in the bilateral free end of the mandible with p=0.03 (p<0,05). Conclusion: The masticatory performance of pre-elderly women is better than the elderly in the bilateral free end of the mandible. KEYWORDmastication performance, bilateral free end, pre-elderly, elderly, view gum.

Abstract

ABSTRAKPendahuluan: Kehilangan seluruh gigi atau edentulous berdampak terhadap mastikasi. Secara objektif, fungsi mastikasi dapat dinilai melalui performa mastikasi yang dapat dipengaruhi oleh kondisi laju alir saliva. Penuaan mengakibatkan perubahan pada fisiologis, seperti penurunan laju alir saliva dan kadar hormon estrogen pada wanita. Tujuan penelitian mengetahui nilai rerata laju alir saliva dan performa mastikasi serta hubungan laju alir saliva dengan performa mastikasi pada wanita pra lansia dan lansia pemakai gigi tiruan lengkap. Metode: Jenis penelitian deskriptif analitik dengan rancangan penelitian cross sectional. Teknik pengambilan sampel adalah non-probability sampling dengan teknik purposive sampling. Data laju alir saliva didapatkan menggunakan permen karet bebas gula dan performa mastikasi menggunakan metode pencampuran warna kemudian dianalisis menggunakan software View Gum. Total sampel wanita pra lansia dan lansia berjumlah 20 sampel, masing-masing terdiri dari 10 sampel dan data dianalisis dengan uji korelasi Spearman. Hasil: Nilai rerata laju alir saliva wanita pra lansia pemakai gigi tiruan lengkap lebih tinggi yaitu 1,88 ml/menit daripada lansia dengan nilai 1,54 ml/menit, nilai rerata performa mastikasi wanita pra lansia pemakai gigi tiruan lengkap lebih baik yaitu 57,30% daripada lansia dengan nilai 58,10%. Simpulan: Terdapat hubungan antara laju alir saliva dengan performa mastikasi wanita pra lansia dan lansia pemakai gigi tiruan lengkap. Keluhan ketidaknyamanan pada lansia saat memakai gigi tiruan lengkap dapat diberikan edukasi oleh dokter gigi untuk menjaga kondisi laju alir saliva pada lansia dan memperbaiki masalah retensi pada gigi tiruan lengkap.Kata kunci: Laju alir saliva, performa mastikasi, usia, gigi tiruan lengkap Correlation between saliva flow rate and mastication performance in pre-elderly and elderly women wearing complete dentures: cross-sectional studyABSTRACT Introduction: Loss of all teeth or edentulousness has an impact on mastication. Objectively, masticatory function can be assessed through masticatory performance which can be influenced by saliva flow rate conditions. Aging causes physiological changes, such as a decrease in saliva flow rate and estrogen hormone levels in women. The aim of the research was to determine the mean value of saliva flow rate and masticatory performance as well as the relationship between salivary flow rate and masticatory performance in pre-elderly and elderly women who wear complete dentures at RSGM USU. Method: Type of research is a descriptive analytical type with a cross sectional research design. The sampling technique is non-probability sampling with purposive sampling technique. Saliva flow rate data was obtained using sugar-free chewing gum and mastication performance using the color mixing method was then analyzed using View Gum software. The total sample of pre-elderly and elderly women was 20 samples, each consisting of 10 samples and the data were analyzed using the Spearman correlation test. Results: The average value of saliva flow rate of pre-elderly women who wear complete dentures is higher, namely 1.88 ml/minute, than the elderly with a value of 1.54 ml/minute, the average value of mastication performance of pre-elderly women who wear complete dentures is better, namely 57. 30% more than the elderly with a value of 58.10%. Conclusion: There is a relationship between salivary flow rate and the masticatory performance of pre-elderly women and elderly women who wear complete dentures. Complaints of discomfort in the elderly when wearing complete dentures can be given education by dentists to maintain the saliva flow rate in the elderly and improve retention problems in complete dentures.Keywords: salivary flow rate, masticatory performance, age, complete denture
