Abstract

Formulir ringkasan masuk dan keluar berisi informasi identitas pasien, cara penerimaan pasien melalui, cara masuk, dikirim oleh, serta berisi ringkasan data saat pasien dipulangkan. Berdasarkan hasil studi pendahuluan di Rumah Sakit X, dapat diketahui jika masih terdapat ketidaklengkapan pengisian formulir ringkasan masuk dan keluar sebesar 20,30%. Tujuan penelitian yaitu untuk menganalisis faktor penyebab ketidaklengkapan pengisian ringkasan masuk dan keluar. Jenis penelitian yaitu kualitatif, dengan subjek penelitian yaitu 4 admisi rawat inap dan 4 DPJP. Pengumpulan data menggunakan wawancara mendalam (in-depth interview), observasi, dan dokumentasi. Hasil penelitian pada faktor individu yaitu usia admisi dan DPJP termasuk kategori usia produktif, sebagian informan memiliki masa kerja lama, tingkat pendidikan yang belum sesuai kualifikasi dan berpengetahuan kurang terkait kelengkapan pengisian ringkasan masuk dan keluar. Faktor psikologi yaitu admisi dan DPJP memiliki persepsi yang baik, sebagian informan memiliki sikap yang kurang terkait kelengkapan pengisian ringkasan masuk dan keluar, seluruh informan belum mengikuti pelatihan pengisian rekam medis dan tidak adanya sistem reward serta punishment. Faktor organisasi yaitu sudah terdapat SPO  dan  dilaksanakan sosialisasi namun masih berjalan kurang maksimal serta pelaksanaan monitoring dan evaluasi yang kurang optimal. Kesimpulan dari penelitian ini yaitu pendidikan yang belum sesuai kualifikasi, pengetahuan dan sikap yang masih kurang, tidak dilakukannya pelatihan, tidak adanya sistem reward/punishment, pelaksanaan sosialisasi, serta monitoring dan evaluasi yang kurang optimal sehingga disarankan untuk melakukan penempatan petugas sesuai dengan kualifikasinya, memberikan pelatihan, reward, dan punishment kepada petugas, melakukan sosialisasi secara periodik serta lebih meningkatkan pelaksanaan monitoring dan evalusi terhadap kinerja admisi dan DPJP.Kata kunci: ketidaklengkapan; rawat inap; ringkasan masuk dan keluar

Abstract

Based on the results of observations at the Bluto Public Health Center, it is known that there was an incompleteness of outpatient medical records with a percentage of 8.2% with the number of medical records that are not completely filled as many as 22 files from 267 files. This study aims to analyze the factors causing the incompleteness of outpatient medical records at the Bluto Public Health Center based on Gibson's performance theory, namely individual factors, organizational factors, and psychological factors by prioritizing problems using the Urgency, Seriousness, Growth (USG) method and improvement efforts through brainstorming. This type of research was qualitative research with data collection methods through interview, observation, documentation, and brainstorming. The results of the study were based on individual factors, namely the experience of medical officers who were still short and most medical officers have never attended training on medical records. Organizational factors, namely there was no control card, obstacles to computer networks, the implementation of Standard Operating Procedures (SOP) that were not optimal, and officers had many responsibilities beyond the main duties in the job description. Psychological factors, namely that there were no punishment for officers who were not complete in filling out the medical records and the officer's lack of accuracy when filled out the outpatient medical record. As for efforts to solved the problem, the medical officers can routinely re-check the completeness of the medical records before it returns to the Medical Record Unit, conduct guidance and training to improve the skills of officers, reduce the use of facilities with high electrical power, plan the budget for repairing electrical installations, routine socialization of Standard Operating Procedures (SOP) and the provision of Standard Operating Procedures (SOP) in each room in the service unit.

Abstract

The results of the preliminary survey, it was found that the outpatient unit had not performed the destruction of medical record since the community health centre was established. According to administrative data, the health centre was established on August 1, 1989, and the destruction was supposed to be performed 16 times in 32 years. This study was to identify problems using the five elements of management: man, money, method, material, and machine. The study was qualitative with the study subjects consist a medical records director, registration officer, and filing officer. Data was collected using observation, interviews, and documentation techniques. The results of the study found that the 'man' factors that caused the unperformed retention and destruction of inactive medical record were the lack of knowledge about the destruction, the educational background of two officers with high school diplomas and one nursing vocational high school diploma, and the officers who had never attended training related to the destruction. The causative 'method' factor was the absence of SOP (standard operative procedure) regarding the destruction of medical records. The 'money' factor included the lack of budget to destruct the medical record files. The 'machine' factor was the unavailability of medical record destruction equipment. The 'material' factor was the absence of a medical record file storage rack. It is suggested to use recommendations from several articles that are nationally accredited.
