Abstract

Background: The diabetes mellitus cases have significantly increased in Indonesia over recent years. Health education for patients has often been carried out; however, education using self-instructional methods, which provided self-learning to solve problems by adjusting the patient's ability to improve self-care behaviors, has not been widely used.Purpose: This study aimed to analyze the self-care behaviors of diabetic patients with the application of self-instructional training.Methods: This research was a quasi-experimental study with a non-equivalent pretest-posttest with a control group design. The participants were 73 diabetic patients in the out-patient units selected by a purposive sampling technique, and divided into two groups: the intervention group (n=37) and the control group (n=36). The intervention group received a self-instructional training program which was carried out in two sessions using a booklet; each session lasted for 45 minutes. The Summary of Diabetes Self-Care Activities (SDSCA) questionnaire was used to collect diabetic patients' self-care behavior data. Wilcoxon and Mann-Whitney tests were employed for data analysis.Results: Self-care behaviors of patients with diabetes mellitus increased before and after the training in both groups (p=0.000). However, the increase in the intervention group was higher than that in the control group, from 46.46±5.014 to 58.03±7.320 and from 47.78±4.929 to 51.64±6.406, respectively. There was also a significant difference in the self-care behaviors of diabetic patients between the intervention group and the control group (p=0.000).Conclusion: Self-instructional training significantly improves self-care behaviors of diabetic patients. Therefore, self-instructional training can be considered to apply in the clinical setting for improving self-care behaviors of diabetic patients to prevent complications, and for enhancing nursing care of diabetes mellitus.

Abstract

Aims: Even though the prevalence of type 2 diabetes mellitus in Indonesia continues to increase, research data on glycemic management, which is the primary strategy for reducing complications of diabetes mellitus, is still limited. In this study, an evaluation of glycemic control status and related factors was conducted in adult patients with type 2 diabetes mellitus. Methods: This research was a cross-sectional study with 115 type 2 diabetes adults who came to the Hospital in Kediri for treatment. Data was collected using the UK Diabetes and Diet Questionnaire (UKDDQ) questionnaire, which consists of 24 questions and the Godin Leisure-Time Exercise Questionnaire (GLTEQ), along with a blood glucose check between July and August 2023. The glycemic level was assessed using a random blood glucose level, and 'poor glycemic control' was defined when the blood glucose level was >200 mg/dL. SPSS was used for descriptive analyses and inferential statistics by chi-square test to analyze the relationship between patients glycemic control status and its relevant risk factors, including demographics, dietary control, and physical activity. Results: Only 26.1% of type 2 diabetes mellitus patients at Aura Syifa Kediri Hospital have good glycemic control. In bivariate analysis, gender, education, control status of diet, and physical activity (P values 0.05) were significantly correlated with glycemic control. Meanwhile, age, marital status, employment status, and duration of diabetes mellitus (p-value> 0.05) were not associated with glycemic control. Conclusion: The prevalence of good glycemic control in type 2 diabetes patients remains low. Based on this research, we found that gender, education, diet control, and physical activity were influencing factors in this problem. Publicity and education regarding the importance of glycemic control should be developed for type 2 diabetes patients, and targeted interventions should be made for risk groups.

Abstract

Aims: Stroke can lead to varying outcomes, ranging from recovery with disability, death, to surviving with recurrent strokes. Post-stroke patients require special attention to improve self-care behaviors. Stroke empowerment education is an educational approach given to stroke patients with an empowerment-focused approach centered on the patient. This study aimed to determine the effect of stroke empowerment education on the self-care of post-stroke patients as an effort to prevent recurrent strokes at RSUD Gambiran Kota Kediri. Methods: The research design used was a Quasi-Experiment with a pre-post test design with a control group. Sampling was done using accidental sampling technique, resulting in 98 respondents. Data collection on self-care was conducted using the modified version of The Subjective Self-Care Performance Scale questionnaire. Results: The results of the Wilcoxon Signed Ranks Test showed significant improvement (p = 0.000) in self-care among post-stroke patients after Stroke Empowerment Education. Conclusion: This study concluded that there was an effect of stroke empowerment education on improving self-care among post-stroke patients as an effort to prevent recurrent strokes at RSUD Gambiran Kota Kediri. As a recommendation, the results of this study can be continued as an intervention in hospitals to enhance services for post-stroke patients to improve self-care as an effort to prevent recurrent strokes.

Abstract

Background: Diabetes mellitus (DM) is a chronic condition with severe effects. Diabetes patients are advised to adopt various self-management decisions and participate in care activities to improve glycemic control. Self-care management strategies assist people with diabetes in making decisions and behaviors that improve health outcomes. Self-care management strategies can be the basis for limiting and preventing serious diabetes-related consequences and financial expenses that emerge from this disease problem. Abstract

Diabetes melitus (DM) terjadi karena gangguan metabolisme disebabkan karena gangguan sekresi insulin atau aktivitas insulin, yang mengakibatkan tubuh tidak cukup memprodukasi glukosa untuk energi. Hiperglikemia dan hipoglikemia pada pasien DM merupakan komplikasi akut yang dapat terjadi secara tiba-tiba, dapat memperburuk keadaan, bahkan dapat mengakibatkan kematian. Tingginya prevalensi dan komplikasi DM seperti hiperglikemia dan hipoglikemia berkaitan erat dengan pengetahuan dan sikap penyandang DM yang kurang tepat. Tujuan kegiatan pengabdian ini untuk meningkatkan pengetahuan dan sikap penyandang DM tentang manajemen self-care DM. Metode kegiatan dengan memberikan edukasi kesehatan dengan media booklet dan materi yang disampaikan meliputi definisi, tanda gejala, penyebab, dampak dan manajemen self-care DM. Jumlah peserta sebanyak 30 orang. Hasil kegiatan pengabdian ini adalah pengetahuan peserta sebelum diberikan edukasi sebagian besar kurang (56.7%), sedangkan sesudah diberikan edukasi hampir seluruh peserta adalah baik (83.3%. Pada variabel sikap pada penyandang  DM sebelum edukasi Sebagian besar adalah negative (63.3%), sedangkan sesudah edukasi hampir seluruh peserta adalah positif (80%). Diharapkan dengan adanya peningkatan pengetahuan dan sikap pada penyandang DM diharapkan dapat meningkatkan kontrol  gula darah pada pasien sehingga komplikasi jangka panjang yang dikaibatkan karena ketidakstabilan gula darah dapat dicegah.

Abstract

There are many factors and variables that influence the prognosis in patients with severe head injuries make determining the prognosis for patients with severe head injuries become difficult. There are two parameters that can predict the prognosis of severe head injury patients with optimal full outline of unresponsiveness score (FOUR score) and brainstem sign score (BSS). FOUR scores can provide a wealth of information about the scale of neurological signs that are quick and easy used for unconcious patients. BSS is an assessment to predict the death of a person permanently to determine whether there is a physiological function that is not function normally. The aimed of this study was to determine ratio of FOUR score and BSS in determining the prognosis of patients with severe head injury at RSUD Gambiran Kediri. The type of study was cross sectional approach. The sampling technique used consecutive sampling technique and obtained a sample of 60 people. The data was analyzed by using chi-square test for comparison of FOUR score and BSS in determining the prognosis of severe head injury patient at RSUD Gambiran Kediri. Based on the results of study by chi-square test showed that p FOUR score = p BSS = 0,004, which meant the FOUR score and BSS was determined the prognosis of patients with severe traumatic head injury at RSUD Gambiran Kediri. The study showed that there was no difference between the FOUR score and BSS in determining the prognosis of severe head injury patient at RSUD Gambiran Kediri but FOUR score could be used in incubated or ventilated patients. Based on the results of this study, it is advisable to use scoring method more easily and according to the condition of the patient is with the installed ventilator or non ventilator.
