Abstract

Hypertension is one of the non-communicable diseases (PTM) which is a very serious health problem both in the world and in Indonesia and is the cause of death which reaches 6.7% of the population at all ages in Indonesia. Dyspepsia is a health problem that is often encountered in daily practice. It is estimated that nearly 30% of cases found in general practice and 60% in gastroenterology practice are dyspepsia. A 41-year-old female patient came to the health center with complaints of dizziness, nausea, vomiting and cold sweat. The patient has a history of hypertension since 2013. In addition, patients also often complain of pain in the pit of the stomach. Complaints are felt repeatedly. Patients do not routinely take antihypertensive medication and do not routinely control and do not follow a healthy lifestyle. Patients have a desire to know more about the patient's illness and want to recover. The patient's knowledge about the disease is lacking, as well as curative treatment patterns, poor diet, rarely exercise and moderate physical activity. Diagnosis and management of these patients has been carried out in a holistic, patient-centered, family approach based on evidence-based medicine and there have been changes in the knowledge and behavior of patients and families after the intervention.

Abstract

Hypertension is one of the biggest causes of morbidity in the world, often referred to as the silent killer. The prevalence of hypertension in Indonesia in 2018 based on Basic Health Research (Riskesdas) with a population of approximately 260 million increased by 34.1% compared to 2013. According to data from the Lampung Provincial Health Office in 2021, the prevalence of hypertension in South Lampung is 24.8% of the total population aged ≥ 15 years is 762,894 people. The purpose of this case report is to implement holistic and comprehensive family doctor services by identifying risk factors, clinical problems, and patient management based on Evidence Based Medicine with a patient centered and family focused approach. This study is a case report. Primary data were obtained through autoanamnesis, physical examination, and home visits. Secondary data were obtained from the patient's medical record. Mr. M, 59 years old, had a chief complaint of difficulty sleeping since 1 day ago. Internal risk factors in the patient are male gender, 59 years old, a history of smoking since 39 years ago, a mother with a history of hypertension, a curative treatment pattern, and the patient's lack of knowledge about: hypertension classification where the patient does not know he is included in hypertension urgency; risk factors that cause the disease; complications that may occur; how to control the disease. External risk factors in the patient are: lack of family knowledge about the patient's disease, risk factors, complications, and prevention and control of the disease in the family and economic stress due to crop failure losses. Diagnosis and management of patients have been carried out holistically, patient centered, family focused, and community oriented. Changes in the clinical, knowledge, and behavior of patients and their families occurred after the intervention was given.

Abstract

The elderly group is included in the vulnerable population because of the process of decreasing the ability to avoid disease and exposure to risk factors for chronic diseases such as Diabetes Mellitus (DM). DM can increase the risk of tuberculosis (TB) infection three times greater than the normal population, and can also increase the risk of reactivation of TB and latent TB. The study conducted was a case report. This case report aims to implement evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and management with a patient-centered approach and a family approach. Primary data were obtained through anamnesis and physical examination by making home visits, filling in family folders, and filling in patient files.

Abstract

Asthma is a chronic inflammatory disorder of the airways that causes bronchial hyperreactivity that causes symptoms such as wheezing, coughing, shortness of breath and chest tightness, especially at night and early morning, which are generally reversible with or without treatment. The application of family doctor services based on evidence based medicine to patients by identifying risk factors, clinical problems, and management based on a problem solving framework with a patient-centered and family approach. This study is a Case Report. Primary data were obtained through history taking, physical examination, home visits to complete family and psychosocial data, as well as the environment. Assessment based on a holistic diagnosis from the beginning, process and end of the study quantitatively and qualitatively. In this patient, pharmacological and non-pharmacological interventions were carried out in the form of education in preventing asthma attacks in patients. Patient An. S, aged 12 years, had the main complaint of shortness of breath accompanied by coughing up phlegm at night. The patient was diagnosed with bronchial asthma and was already on medication. The patient's internal risk factor is having knowledge about the disease and the patient's home environment that is exposed to road dust. The external risk factor that affects is the state of the house that has but the windows are rarely opened. For the patients, it was done in the form of counseling about asthma, counseling on cleaning the home environment from dust, using masks when doing activities, and taking medication regularly. After the intervention, there was a change in the knowledge of the patient and his family.

Abstract

Hypertension is a condition where systolic blood pressure is ≥ 140 mmHg and/or diastolic is ≥ 90 mmHg. Dyspepsia is a collection of symptoms that lead to upper digestive tract disorders including pain or discomfort in the gastro-duodenal area, burning sensation, fullness, early satiety, nausea, and vomiting. This case report identifies risk factors, clinical problems, and provides management to patients by implementing holistic family doctor services based on evidence based medicine through a patient centered and family approach. Primary data were obtained through autoanamnesis, physical examination, and home visits to complete family data, psychosocial data, and environmental data. Secondary data was obtained from patient medical records at the Puskesmas. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study both quantitatively and qualitatively. Holistic management was carried out, namely intervention using calendars, posters, and cups on a 48-year-old female patient who complained of headaches, heartburn, and nausea. The patient's internal risk factors include pre-elderly age, curative treatment patterns, overweight BMI, infrequent exercise & physical activity, inappropriate diet & eating habits, and wrong perceptions about the disease. The patient's external risk factors include stress, lack of family support and knowledge, and family history of hypertension. The evaluation obtained increased understanding of the disease and changes in behavior that have an impact on the success of therapy.
