Abstract

The elderly group is included in the vulnerable population because of the process of decreasing the ability to avoid disease and exposure to risk factors for chronic diseases such as Diabetes Mellitus (DM). DM can increase the risk of tuberculosis (TB) infection three times greater than the normal population, and can also increase the risk of reactivation of TB and latent TB. The study conducted was a case report. This case report aims to implement evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and management with a patient-centered approach and a family approach. Primary data were obtained through anamnesis and physical examination by making home visits, filling in family folders, and filling in patient files.

Abstract

Asthma is a chronic inflammatory disorder of the airways that causes bronchial hyperreactivity that causes symptoms such as wheezing, coughing, shortness of breath and chest tightness, especially at night and early morning, which are generally reversible with or without treatment. The application of family doctor services based on evidence based medicine to patients by identifying risk factors, clinical problems, and management based on a problem solving framework with a patient-centered and family approach. This study is a Case Report. Primary data were obtained through history taking, physical examination, home visits to complete family and psychosocial data, as well as the environment. Assessment based on a holistic diagnosis from the beginning, process and end of the study quantitatively and qualitatively. In this patient, pharmacological and non-pharmacological interventions were carried out in the form of education in preventing asthma attacks in patients. Patient An. S, aged 12 years, had the main complaint of shortness of breath accompanied by coughing up phlegm at night. The patient was diagnosed with bronchial asthma and was already on medication. The patient's internal risk factor is having knowledge about the disease and the patient's home environment that is exposed to road dust. The external risk factor that affects is the state of the house that has but the windows are rarely opened. For the patients, it was done in the form of counseling about asthma, counseling on cleaning the home environment from dust, using masks when doing activities, and taking medication regularly. After the intervention, there was a change in the knowledge of the patient and his family.

Abstract

Hypertension is a condition where systolic blood pressure is ≥ 140 mmHg and/or diastolic is ≥ 90 mmHg. Dyspepsia is a collection of symptoms that lead to upper digestive tract disorders including pain or discomfort in the gastro-duodenal area, burning sensation, fullness, early satiety, nausea, and vomiting. This case report identifies risk factors, clinical problems, and provides management to patients by implementing holistic family doctor services based on evidence based medicine through a patient centered and family approach. Primary data were obtained through autoanamnesis, physical examination, and home visits to complete family data, psychosocial data, and environmental data. Secondary data was obtained from patient medical records at the Puskesmas. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study both quantitatively and qualitatively. Holistic management was carried out, namely intervention using calendars, posters, and cups on a 48-year-old female patient who complained of headaches, heartburn, and nausea. The patient's internal risk factors include pre-elderly age, curative treatment patterns, overweight BMI, infrequent exercise & physical activity, inappropriate diet & eating habits, and wrong perceptions about the disease. The patient's external risk factors include stress, lack of family support and knowledge, and family history of hypertension. The evaluation obtained increased understanding of the disease and changes in behavior that have an impact on the success of therapy.
