Abstract

Banyak tenaga kesehatan profesional yang mengalami burnout selama pandemi COVID-19, termasuk psikiater sebagai pemberi layanan kesehatan jiwa. Meningkatnya jumlah kasus, konsulan, rujukan, serta keterbatasan sumber daya membuat karakteristik kerja psikiater penuh tekanan dan menurunkan kualitas kehidupan profesional psikiater. Penelitian ini bertujuan mengeksplorasi karakteristik pekerjaan dan kualitas hidup profesional psikiater selama pandemi COVID-19. Responden dalam penelitian ini berjumlah sepuluh orang psikiater anggota Perhimpunan Dokter Spesialis Kedokteran Jiwa Indonesia (PDSKJI) cabang Malang yang bertugas dalam pelayanan klinis selama pandemi COVID-19 dan dipilih menggunakan purposive sampling. Penelitian ini menggunakan pendekatan fenomenologi interpretatif dengan metode wawancara mendalam. Didapatkan dua tema terkait karakteristik pekerjaan psikiater selama pandemi COVID-19, yaitu peran penting dalam mendukung kesejahteraan pasien, keluarga pasien, staf tenaga kesehatan lain yang berisiko, dan masyarakat, serta dualitas tanggung jawab menghadapi masalah neuropsikiatri maupun kondisi medis umum. Didapatkan tiga tema terkait kualitas hidup psikiater selama pandemi COVID-19, yaitu dilema antara hak atas keselamatan dan tanggung jawab, kepuasan kerja, serta kelelahan dalam bekerja. Kesimpulannya, karakteristik kerja psikiater selama pandemi COVID-19 cukup kompleks karena memainkan peran penting bagi pasien dan keluarganya, tenaga kesehatan lain, serta masyarakat, dan juga menghadapi dualitas tanggung jawab. Kualitas hidup psikiater selama pandemi COVID-19 dipengaruhi oleh dilema antara tanggung jawab dan hak atas keselamatan. Psikiater menemukan rasa kepuasan, walaupun secara umum kepuasan kerja menurun karena tidak bisa melakukan pelayanan maksimal. Meningkatnya rasa cemas, menurunnya kepuasan kerja, serta peningkatan beban kerja menimbulkan kelelahan pada psikiater, sehingga menurunkan kualitas hidup profesional psikiater.

Abstract

Introductions: Depressive disorders are expected consequences of experiencing child maltreatment. Many depressive disorders can occur with or without psychosis, which has different implications for treatment and prognosis. This condition raises the challenge of treating depression in maltreated adolescents because the patient has inadequate family and social support. Consequently, it is difficult for the patient to undergo psychotherapy that involves family.  Case: A 17-year-old female was diagnosed with psychotic depression. The patient had symptoms of depression, accompanied by hallucinations, since five years ago. The patient was hospitalized for suicidal ideation and food refusal for days. Patient had a history of child maltreatment by family and experienced bullying since elementary school. Patient believes that her family, especially her mother, had hated her from the beginning. Discussions: After stabilization of the patient's general condition, psychopathology exploration was done together with selection of appropriate treatment. Combined Cognitive Behavioral Therapy (CBT), Selective Serotonin Reuptake Inhibitor (SSRI) and atypical antipsychotic treatment are the evidence-based treatments for psychotic depression in maltreated adolescents. Studies involving patients with psychotic depression which were treated by combination of sertraline and olanzapine showed significant improvement of depression and psychotic symptoms and reduced the risk of relapse over 36 weeks, compared to sertraline plus placebo. Conclusion: Combination of SSRI, atypical antipsychotic and CBT with a trauma-informed approach should be considered as treatment for psychotic depression in maltreated adolescents.

Abstract

Introduction: Neuroleptic Malignant Syndrome (NMS), a neuropsychiatric emergency, can develop in patients with antipsychotic medications. This is a matter of concern due to the high frequency of psychotic diseases in Indonesia, affecting an estimated 2.6 million people. This paper will examine a case of NMS who exhibited various risk factors and clinical manifestations, which happened in Kediri, a city in Indonesia that lacks of dedicated mental hospital or intensive care unit for mental health. Method: In this paper, we reveal about a case report about a 63-year-old man presented to the emergency room with abrupt onset of fever and cognitive impairment. He took olanzapine and clozapine for schizophrenia, which was not often treated. The patient had gait disturbances, pronounced tremors, rigidity, and autonomous movement of the mouth. Examination results were blood pressure of 88/55 mmHg, pulse 123x/minute, respiratory rate 24x/minute, body temperature 42°C. Supporting investigation results showed leukocytosis, hypokalemia, pneumonia and cardiomegaly. Initial management included discontinuation of neuroleptic drugs, administration of oxygen, rehydration, and symptomatic medications. Results: This is a rare case of NMS in geriatric patient happened in a rural area, Kediri, a small city in Indonesia which experiences higher temperatures in comparison to other cities. This case is quite complex due to the treatment options, a history of poor drug compliance, elderly conditions and comorbidities. Discuss: For both confirmed and suspected instances of NMS, basic treatment suggestions include stopping antipsychotic medication, continuously monitoring vital signs, minimizing extremes in temperature, introducing fluids, correcting electrolyte imbalances, and providing other supportive measures as needed Conclusion: NMS should be closely monitored in psychosis patients with a history of noncompliant medication use and concomitant conditions. To save lives, prompt and adequate medical attention is essential.
