Abstract

Background: Asthma is a chronic inflammatory disease of the respiratory tract characterized by narrowing of the airways, resulting in symptoms of shortness of breath, wheezing, and decreased oxygen saturation. This condition requires prompt and appropriate management, particularly in the Emergency Department (ED). In addition to pharmacological therapy, non-pharmacological nursing interventions such as pursed lip breathing and the tripod position can be used to help increase pulmonary ventilation and improve oxygenation in asthmatic patients. Purpose: To analyze the application of innovative interventions such as pursed lip breathing and the tripod position to improve oxygen saturation in asthmatic patients. Method: This study used a descriptive case study design with a nursing action evaluation method. The study was conducted from December 2025 to January 2026 at the Emergency Department (ED) of A.M. Parikesit Tenggarong Regional General Hospital. The study subjects consisted of one intervention patient (Mrs. S) who received PLB and tripod position training, and two control patients (Mrs. A and Mrs. V) who received standard care and medical therapy. Data were collected through interviews, observations, physical examinations, and monitoring of vital signs and oxygen saturation for 4 hours. Results: The combination of pursed lip breathing and the tripod position increased oxygen saturation in asthma patients. In the intervention patient (Mrs. S), oxygen saturation increased by 2%, from 94% to 96%. Meanwhile, in control patients, the increase in oxygen saturation was relatively stable, with an average increase of 1% in Mrs. A and 0% in Mrs. V. Furthermore, intervention patients also experienced a decrease in shortness of breath and an improvement in breathing patterns. Conclusion: The pursed lip breathing and tripod position interventions have proven effective in increasing oxygen saturation in asthma patients. The combination of these two techniques is a safe, easy, and applicable non-pharmacological intervention for use in the emergency department (ED) for patients who are conscious and able to follow instructions.   Keywords: Asthma; Oxygen Saturation; Pursed Lip Breathing; Tripod Position.
