Abstract

Background: Traumatic fingertip injuries are the most common injuries of the hand. Several million estimated visits to primary care pro­vi­ders and the emergency department occur each year. But the handling is often not done pro­per­ly and comprehensively. Reconstruction of the fingertip must be done as soon as possible in the acute stage, resulting in better outcomes in the future. Aim of the surgery was to restore as much finger length, preserve sensation finger­tip properties and achieve normal nail growth, and cosmetics appearance as well.Case presentation: Three cases of emergency patients with fingertip injury treated at the hospital in the acute setting. All of these pati­ents experienced work injuries and were imme­diately taken to the emergency department.Results: The debridement and skin coverage reconstruction were carried out. Functional and aesthetic outcome were a postoperative assessment.Conclusion: The right choice of treatment and good wound care will produce a better outcome.Keywords: traumatic injury, fingertip injury, skin coverageCorrespondence: Tito Sumarwoto. Abstract

ABSTRACT Background: When pain, deformity and instability compromise wrist function, wrist stabilization by means of fusion is a procedure of recognized validity. The goal of wrist arthrodesis is to provide the patient with a stable wrist for power grip and the predictable relief of pain while sacrificing wrist motion. The radioscapholunate (RSL) fusion is one of salvage procedure indicated in the case of traumatic or degenerative osteoarthritis of the radiocarpal joint, involving the lunate facet of the radius. RSL-fusion reduced total wrist motion (3 MC/Rad) in the F/E axis to an average of 49% of F/E in the unfused wrist but preserves midcarpal joint motion while alleviating pain. Case Report: We report a 43-year-old male patient with pain on his left wrist. 10 months before admission, he felt down from stairs with his left hand bear his body weight. He broke his wrist and got surgery at that time. Because still feel pain and his index finger can’t fully extensed, he brought to Soeharso orthopaedic hospital. Here, we have done RSL-fusion using cross pinning K-Wire to reduce pain, distal scapoid excision to optimizing radial angulation and release superficial flexor tendon index finger of left hand. Discussion: With Secondary OA of left wrist post traumatic with neglected radiocarpal dislocation and superficial flexor tendon contracture due to entrapment index finger of left hand post RSL-fusion, distal scapoid excision and release contracture can reduce pain at radiocarpal joint and improve range of movement. Abstract

Background: Drop hand, resulting from radial nerve injuries, causes functional and structural disabilities. Tendon transfer becomes the last option when nerve repair fails to achieve restoration. The most popular method is the Jones tendon transfer to restore wrist dorsiflexion and extension of the thumb and fingers. The Disabilities of the Arm, Shoulder, and Hand (DASH) score measures functional outcomes in patients with upper extremity injuries. This study aimed to assess the functional outcome and evaluate the DASH score in drop hand patients who underwent Jones tendon transfer surgery at Prof. Dr. R. Soeharso Orthopaedic Hospital Surakarta. Methods: Data collected from medical records between January 2014 and June 2016 were evaluated based on functional outcome and DASH score. Eleven patients were included, comprising ten male patients (90.9%) and one female patient (9.1%). The mean age of the patients was 27.4 years. The right arm was the most frequently injured, accounting for eight patients (72.7%), while the left arm was injured in three patients (27.3%). Results: The DASH score evaluation indicated minimal disability in ten patients (90.9%) and moderate disability in one patient (9.1%). The average score was 12.48, indicating that most patients could cope with most daily living activities postoperatively.Conclusions: Surgical intervention for drop hand due to radial nerve palsy using the Jones tendon transfer technique yielded a satisfactory functional outcome based on the DASH score.
